3rd Street Youth Center & Clinic is a community-based agency providing youth in the Bayview Hunters Point with medical and behavioral health services that encourage them to make decisions that support their health, safety, and development. 3rd Street Youth Center & Clinic also manages a number of successful Transitional Age Youth (TAY) housing stabilization programs and one of the city's few Youth Access Points.
The Housing Case Manager - Rapid Rehousing Parenting TAY will provide comprehensive case management to TAY youth placed into housing via San Francisco’s Coordinated Entry. The Housing Case Manager – Rapid Rehousing Parenting TAY will assist clients with securing needed documentation to move into housing, provide linkages and referrals to resources etc. They will also provide wrap-around case management services with a harm reduction model to ensure the clients long-term housing retention and improved well-being. The Housing Case Manager Rapid Rehousing Parenting TAY will work closely alongside the Housing Navigator to help clients secure hosing.
Duties & Responsibilities:
The following is not designed to cover or contain a comprehensive listing of activities, duties or responsibilities that are required of the Housing Case Manager - Rapid Rehousing Parenting TAY. Duties, responsibilities, and activities may change, or new ones may be assigned as needed.
● Individual case management & oversight to TAY referred from the Coordinated Entry System, including twice monthly home visits and in-person visits.
● Create housing retention plans surrounding self-sufficiency.
● Resource brokerage and referral as needed to community resources for behavioral health, employment, education, families, public benefits, legal, low-income housing, In Home Support services (IHSS), and etc.
● Case coordination and advocacy within agency and with other Community Based Organization’s CBO's
● Data collection and case notes entered into all appropriate databases.
● Support Housing Program Manager and Housing Navigators in all aspects of 3rd Street's Rapid Rehousing program
● Designing individual support & transition plans with youth including independent living skills and income stability.
● Attend community, city, & regional housing meetings
● This role requires a strong focus on administrative tasks to support housing services, including maintaining detailed client records, managing physical files, completing extensive documentation, and ensuring accurate data entry in systems such as HMIS for program compliance and reporting.
● The ideal candidate will possess strong organizational skills, attention to detail, and the ability to manage multiple deadlines. Proficiency in data management systems, Microsoft Office (especially Excel and Word), and effective file organization is essential.
● Candidates should be comfortable handling a high volume of paperwork, organizing both digital and physical records, collaborating with case managers, engaging directly with clients when needed, and adhering to organizational and funding requirements to best support our clients' housing stability.
● Support for staff & youth in 3rd Street programming as needed

Education & Experience:
● Bachelors in related field OR equivalent work
● 2-3 years of experience working with high-risk and homeless youth.
● Experience in using the ONE/Clarity/HMIS System preferred
● Working knowledge of issues facing youth.
● Valid California Driver's License and clean driving record required. Must also have daily access to a car.
● Experience working with homeless youth and with diverse staff, clients, and volunteers.
● Experience with US Department of Housing and Urban Development (HUD) programs a plus
● Experience working with One system (preferred)
● Proficient in Google Suite (preferred)
● Bilingual in Spanish (preferred)

● This position is a salary, exempt and full-time role.
● This position is located in our main office in the Bayview/Hunter's Point district in San Francisco.
● This position reports to the Housing Program Manager.
Compensation and benefits:
● This position has an annual salary of $68,640 and commensurate with experience.
