About the role
Are you a passionate Mental Health Professional interested in working with the most vulnerable? We’re seeking a dynamic & innovative QMHP/T for our expanding Partial Hospitalization Program and Intensive Outpatient programs in Hampton, VA. In this role, you will be responsible for Group Facilitation in our PHP & IOP programs to help them achieve a better life. This is an exciting opportunity to make a lasting impact on the mental health community.
What you'll do
- Develop and maintain Individual Service Plans and Progress Notes on group participants.
- Work with individuals to foster new or improved skills on a group level.
- Case management functions of linking, arranging, and advocating for services and referrals.
- Maintain up-to-date individual records and prepare required session notes in a timely and accurate manner.
- Establish a good working relationship with other public and private agencies/services and interface collaboratively with them.
- Ongoing assessment of treatment needs of individuals.
- Maintain continuous contact with the identified individual and other key stakeholders in planning and service delivery.
- Maintain up-to-date individual records and prepare required/necessary reports in a timely and accurate manner.
- Establish a good working relationship with other public and private agencies/services and interface collaboratively with them.
Qualifications
- Bachelors degree in Psychology, Sociology, Human Services or related field
- Must have experience facilitating groups
- Must qualify as a Qualified Mental Health Professional (QMHP) or in training through the Virginia Department of Health Professions
