About the role: 


This role functions as a remote patient monitoring nurse with an individual patient case load in collaboration with the Brook Health team, accountable for the application of remote clinical nursing practice and patient care management.  There is responsibility to manage the resources within the individual team: human, fiscal and any other resource identified as they apply to the development of process, personnel and compliance. This is a nurse care/case manager type role that requires mindfulness and adherence to regulatory and professional standards, including Health Insurance Portability and Accountability Act (HIPAA) as well as any additional security requirements identified. It is imperative to foster collaborative relationships within all areas of the interdisciplinary workings of the contracted clinic/health care provider and Brook as a partnership. This role/position is largely responsible for patient onboarding, care planning and education for new patients on the Brook platform.


Responsibilities: 


       Work in a tech-first model of healthcare.

       Collaboration, shared decision-making, and partnership across clinical and non-clinical care team   members, including our health coaches.

       Co-manage a population living with one or multiple chronic conditions.

       Collaborate on a panel of members assigned to your care team to provide clinical monitoring of physiological data based on outlined pathways and care coordination based on the member’s care plan.

       Remotely review patient/client data, health information and engage with members through text and voice calls.

       Frequency of contact/ touchpoints determined as patient/client need indicates from daily to monthly to ensure quality outcomes and billing/coding requirements are met.

       Keep a detailed status record (documentation) on members and report out weekly updates.

       Leverage strong time management skills to make impactful judgment calls on member care and balance with daily team meetings and weekly updates.

       Utilize our custom-built care delivery platform, POCAR, and the partner’s EMR to collect data, document member interactions, organize information, track tasks, and communicate with your team and members.

       Foster lasting and trusting relationships to assist members in achieving goals, identifying new needs, and coordinating care.

       Conduct visits to, and build rapport with, partnering providers and their staff

       Effectively articulate the benefits of Brook’s services, enroll eligible members, and educate members on the use of their devices.

       Provide ongoing clinical monitoring to your panel of members in partnership with their provider, making escalation and triage decisions based on their health needs.


Requirements:


     Graduate of an accredited professional nursing program; Associate Degree in Nursing (AAS/AASN, ADN, ASN) required. Current BLS certification preferred

       RN with active, unrestricted CT license required, in addition a MA license is strongly preferred: understanding of scope of practice.

       American Diabetic Association or equivalent education course (professionals) completion within 6 months of hire (subject to course availability).

       Triage certification completion within 6 months of hire. 

       3+ years of experience providing clinical/direct care services, preferably with chronic conditions. Minimum 1 year acute care experience preferred.

       Interpersonal skills necessary to work productively with internal Brook team and external clinical team and support services.

       Possesses active listening skills.

       Planning and critical thinking skills to develop care plans, goals and objectives; implements action plans to meet those goals.

       Analytical ability to identify opportunities for improvements as well as positive problem resolution.

       Teaching abilities to promote optimum education of patients/clients.

       Has current knowledge of health care trends and issues as they  relate to remote monitoring and chronic disease management.

       Computer literate. Proficiency with Microsoft Office, specifically Excel.

       Exceptional triage, coordination, and communication skills.

       Excellent problem solving and organizational skills.

       Experience and comfort working on software platforms and ability to teach to others.

       Experience working with acute care and outpatient Electronic Medical Records (EMR) such as:  Allscripts, MEDENT, Epic, Cerner.
