About the role

Job Summary Statement: 
The purpose of Casa’s “Transcend: Empowerment, Recovery, and Reentry” Program is to reduce relapse and recidivism among Latino reentering citizens living with co-occurring substance abuse and mental health disorders (CODs) by expanding access to integrated, bilingual/bicultural services that provide a bridge to help individuals overcome the pitfalls of homelessness, relapse, and recidivism they face upon release and reentry into the community from incarceration. 
The Reentry Coordinator will be responsible for assisting in the coordination of the day-to day operations of our reentry program, which provides health education and counseling services within a correctional facility and after release from custody. This also includes development of physical and/or virtual communal space, associated programming, and community integration initiatives. 
 
Under the direction of the Senior Associate Director, they will be responsible for coordinating with correctional facilities to transition participants from corrections to a recovery-oriented continuum of care and developing and strengthening systems linkages to expand access to the community resources necessary to lay the groundwork for change, support sustained recovery, and facilitate meaningful participation in the community. S/he will provide supervision and oversight to Reentry Case Managers and work in collaboration with other Casa programs to develop and strengthen systems linkages to enhance and expand access to a recovery-oriented continuum of care, as these are critical to their stability and behavioral health engagement. The Reentry Coordinator will ensure that benchmarks, timelines, goals and objectives are being met and reported as required.
 
As part of the Casa Esperanza, Inc. team, they will play a lead role in providing services in a fully integrated, holistic, and collaborative manner using a team-treatment approach, to ensure the coordination of all client services, both across Casa programs as well as with external providers.

What you'll do

Job Responsibilities:
Program Planning and Service Delivery
  • Work with Associate Director of Community Re-engagement Services to develop and implement outreach strategy for reaching Latinos coming from incarceration with behavioral health disorders
  • Work with Associate Directors of Recovery Support Services to develop schedule for support groups and re-entry clinics
  • Work with RSS team to coordinate social activities, and community re-integration initiatives
  • Represent the agency by providing education and participating in community events, including monthly Reentry Panels in correctional facilities
  • Coordinate with other re-entry services, such as MassHealth’s BH-JI Initiative, as well as other relevant resources available to clients.
  • Coordinate psycho-education groups and re-entry services inside correctional facilities and/or through telehealth for current and potential patients
  • Build collaborative relationships within the community that support a strong recovery system of care, including linkages to bilingual self-help supports and services • Work with residential staff to coordinate internal and external services, i.e. transportation, legal, medical, and housing, employment, and training, etc.
  • Recruit community stakeholders for participating in the Latino Recovery Network to develop a comprehensive, multi-agency approach to improving reentry services and co-facilitate quarterly meetings.
  • Collaborate with BSAS, MassHealth and other relevant stakeholders to continue to develop a sustainable system of care focused on Reentry services and supports
  • Ensure Reentry Specialists collaborate with Leadership and Staff in correctional facilities to develop transition plans for patients leaving correctional facilities and entering Casa’s system of integrated care
  • Advocate for Reentry services, wrap-around supports and community care to specialty courts, parole, probation, DCF, DMH and other state-wide agencies in accordance with Casa policies
  • Collaborate with Leadership to address any barriers to implementation or programs & services
Administrative
  • Review pre-release assessments to ensure eligibility and review transitions plans
  • Ensure ESM data is submitted in accurately and in a timely manner
  • Manage referral process and monitor case assignments to ensure appropriate caseloads
  • Ensure program is meeting state and federal standards of care and reporting requirements
  • Assist in data collection and outcomes reporting, per grant guidelines, parameters and regulations
  • Complete periodic review of client records to ensure all client encounters and other data are documented as required and maintain up-to-date client records in web-based database
  • Maintain up-to-date contacts for key resources, and ensure the team is trained to leverage these organizational linkages to support client access to care
  • Write and submit all reporting as required by Program funding sources in a timely manner, and as required
  • Ensure Patient Satisfaction Surveys are completed
  • Work closely with multidisciplinary team to ensure that client’s treatment and service plans are implemented, and client needs are being met
  • Ensure all documentation, including timesheets, related to billing is reconciled and submitted to appropriate parties in a timely manner
  • Attend all relevant case conferences and staff meetings, including MDT
  • Attend grantee meetings, implementation meetings, and conferences as appropriate or required.
  • Work closely with the Grant Data Collector to ensure program intake and assessment requirements are happening in a timely manner
  • Attend bi-weekly supervision
  • Other duties as assigned and as required
Supervisory Responsibilities
  • Establish and maintain efficient staff workflow to ensure client utilization meet program goals and objectives, including HOC visits and outreach efforts.
  • Ensure all direct staff follow all rules and regulations established in Casa Esperanza’s Employee Handbook, including dress code and time and attendance policies. 
  • Ensure all monthly staff and grant meetings are occurring
  • Provide bi-weekly/weekly supervision to staff and complete corresponding paperwork, and annual staff performance review
  • Work collaboratively with Management Team to support coverage of opening and closing of building
  • Provide on call coverage as needed
  • Other duties and assigned and as required

Qualifications

Knowledge and Skills necessary for the position:
  • Ability to obtain clearance for, and willing to travel to, correctional facilities
  • Bilingual Spanish/English required.
  • Demonstrated supervisory and project management ability
  • Ability to understand and manage moral/ethical boundaries with clients/staff. 
  • Familiar with social, health and cultural issues specific to the Latino population
  • Strong organizational skills
  • Strong verbal and written communication skills
  • Strong interpersonal skills and crisis management skills
  • Able to effectively use computer programs, including web-based database system 
  • Access to vehicle and a valid driver’s license preferred.
Experience required/preferred for the position:
  • Minimum two years’ experience working with individuals with substance use and mental health disorders.
  • Minimum two years’ experience working with justice involved individuals. 
  • Minimum two years’ Case Management experience    
Education/Training required:
  • Bachelor’s degree in social work, psychology, counseling or other Human Services related field preferred (or comparable experience)
  • Certified Alcohol and Drug Counselor/Certified Alcohol Counselor required or willing to obtain within one year of hire
  • CPR certified
Competencies:
  • Spanish Bilingual - Must be able to read and write in Spanish   
Working Conditions/Environment:
  1. Physical Space: Workspace will be in-person, office is in a shared space and will be in multiple buildings on our campus. Position will also require work in HOC (House of Correction) facilities, courts, and other workspaces where are clients needs are located.

This position will also require working with staff and supervisors from other community organizations and the HOC and Justice Department. 
  4. Working Conditions: Must have availability, in-person, M-F 8am – 8pm, and Saturday 8am -12pm. Travel will be required to HOC and courts throughout Massachusetts. Milage Reimbursement will follow federal guidelines. Casa Esperanza follows State Emergency Guidelines for operations closures.

While performing the duties of this job the employee is regularly required to walk, stoop, kneel, crouch or crawl and talk or hear. The employee must occasionally lift and/or move up to 20 pounds. Specific vision abilities required by this job include distance vision, peripheral vision, and depth perception.
