Who is Certintell?
Founded in 2014, Certintell is a Care Management company that offers programs to Medicare, Medicaid, and Dual-eligible patients in partnership with medical clinics, pharmacies, and insurance providers. Certintell acts as a virtual care clinic by providing programs that improve access to care, reduce the cost burden of chronically ill patients, increase revenues to the Health Center, and can improve overall Value-based Payments (VBP) and other relevant outcome-driven ratings (e.g. HEDIS).

Job description
Provides rehabilitation services at the Pediatric and Adolescent Transformative Health (PATH) Clinic. PATH is a 12-member collaborative led by PATH and will provide an innovative structure for a Behavioral Health Home (BHH) serving children, adolescents, and families on the west and southwest sides of Chicago. Evaluates and treats patients using the principles and practices of speech language pathology in accordance with the patient's age, developmental level and diagnosis. Assists clinical manager with administrative duties as assigned.
*This position requires occasional travel to clinics*
Essential Functions (list in order of importance including measures)
- Evaluates patients, conducts diagnostic evaluations, establishes goals and develops individualized treatment plans.
- Provides direct patient treatment based on current speech and language pathology practice and revises treatment plan based on the patient's response.
- Provides counseling and education (as necessary) to patient and family regarding patient's; abilities and needs for assistance and continued therapeutic intervention.
- Consults with physicians on patient's evaluations, progress and complications with treatment.
- Maintains medical and departmental records according to department policies and procedures.
- Communicates with other professional staff and patient's family/support system to provide continuity of care.

- Bilingual English/Spanish a plus
- Experience working with children
- Effective written and verbal communication skills
- Customer Service skills
- SLP-IL License
- CPR Card
- CCC and IDFPR licensure required

Job Type: Contract
Pay: $40.00 - $50.00 per hour
Expected hours: 20 – 30 per week
Schedule:
- 4 hour shift
- Monday to Friday
