Overview:
The Case Care Management Pharmacist is responsible for performing comprehensive medication therapy reviews with eligible Medicare members. The work is detail-oriented, and candidates will be expected to provide sound clinical judgement. Communication skills are top priority as direct patient interactions are a primary function.

·       Assess member’s medication profile during session with member to identify, prevent, and resolve medication-related problems to help members maximize benefits and minimize cost. Complete outreach to other healthcare professionals as necessary

·       Collaborate with internal and external stakeholders to improve overall population health through direct patient interaction, education, and resource utilization.

·       Hours of operation M-F 8AM-6:00PM CST. Potential for weekend and extended evening hours periodically as applicable.

 

Qualifications:

·         Doctor of Pharmacy (PharmD) or Bachelor of Science in Pharmacy degree.

·         Current pharmacist license in good standing at issuing State Board of Pharmacy.

·         1 year of experience as licensed pharmacist.

·         Must be eligible to work in the United States without need for work visa or residency sponsorship.

 

Skills:

·         Prior MTM/ambulatory care experience preferred

·         MTM certification preferred

·         Spanish speaking or multilingual not required but advantageous

·         Confident in making clinical decisions independently.

·         Ability to provide exceptional member experience while documenting interactions efficiently

·         Ability to prioritize tasks and manage multiple projects simultaneously.

·         Motivated self-starter who does not require constant direction or feedback to drive results.

·         Flexibility to evolve in a dynamic working environment.

·         Proficient with learning new computer systems and using multiple monitors.

The pay range for this role is:

55 - 58 USD per hour (Remote)
