Pay: $28.00 - $34.00 per hour

Job description:

Qualifications

• Licensed Practical Nurse currently licensed in the State of Illinois
• Two (2) years of nursing experience required.
• Experience in post-acute care or long-term care preferred.
• Geriatric patient experience required.

Responsibilities

• Develop care plans based on mutual goals with patient, family, physician, and documentation in medication record. Promote patient compliance with care plan and revise the care plan as needed.
• Collaborate with patients, their families/caregivers, healthcare professionals and community resources to develop a comprehensive plan of care that promotes health and meets the patient’s care goals.
• Provide education to improve patient and family health literacy on patient’s chronic conditions.
• Facilitate patient access to appropriate medical and specialty providers as well as other care coordination team support.
• Provide timely communication and follow up with patients, families, and healthcare providers regarding changes in patient condition.
• Serve as a point of contact, advocate, and informational resources for patient, family, care team and community resources as needed.
• Assess the patient’s and family’s unmet health and social needs.
• Help patient and family with medication management and adherence to follow-up appointments.
