About the role


The Registered Nurse (RN) provides comprehensive nursing care and clinical support within a federally qualified health center (FQHC) serving youth in a congregate care and crisis shelter setting. The RN delivers patient-centered care using a trauma-informed, harm-reduction approach and supports coordination of medical, behavioral health, and social services. Responsibilities include direct clinical care, care coordination, and operational support functions, along with additional duties as assigned within the RN scope of practice.


What you'll do

CORE RESPONSIBILITIES – CLINICAL CARE & TRIAGE

· Conduct nursing triage and assessment of acute and chronic conditions

· Support medical providers during clinical visits and procedures

· Conduct daily medical rounds and monitor patient status

· Provide direct patient care, including wound care, point-of-care testing, and health screenings

· Assist with HIV/STI testing, prevention, and patient education

· Recognize and respond to urgent/emergent situations and escalate appropriately

MEDICATION & VACCINE MANAGEMENT

· Administer medications, including vaccines and PrEP, per provider orders

· Support medication management, reconciliation, and adherence monitoring

· Maintain and monitor medication and vaccine inventory, including storage compliance, cold chain management, and ordering

· Support prior authorizations and access to medications

· Verify immunization status and update records, including medication reconciliation

CARE COORDINATION

· Coordinate care with interdisciplinary teams including medical, behavioral health, and residential staff

· Facilitate lab testing, imaging, and follow-up care

· Communicate clinical updates to providers and team members

· Support continuity of care within a congregate shelter environment

DOCUMENTATION & SYSTEMS

· Document patient care in the EHR (eClinicalWorks) in a timely and accurate manner

· Manage lab orders, results tracking, recalls, and follow-up

· Review charts for missing labs, reports, or open orders

· Adhere to all FQHC, HRSA, FTCA, OSHA, and infection control standards

· Participate in quality improvement (QI) initiatives, risk reduction activities, and incident reporting

· Troubleshoot systems issues and participate in quality improvement efforts

Participate in workflow and outcome tracking, including EMR reporting and collaboration with internal and external partners

POPULATION HEALTH & QUALITY

· Support preventive care outreach and chronic disease management

· Assist with quality improvement initiatives and infection control standards

· Provide patient education on treatment adherence and risk reduction


Qualifications

  • Bachelor’s degree in nursing required
  • Current New York State RN license
  • BLS certification required
  • Experience with trauma-informed care and underserved youth populations strongly preferred

SKILLS

  • Strong clinical and technical nursing skills
  • Experience with EHR systems (eClinicalWorks preferred)
  • Ability to work in a fast-paced, interdisciplinary environment
  • Strong communication and organizational skills

The pay range for this role is:

52.50 - 52.50 USD per hour (41st ST - SHELTER)
