This is a part-time, multi-site clinical position tailored for a Nurse Practitioner who excels in geriatric assessment and preventative care. In this role, you will lead Annual Wellness Visits (AWVs) in your region, ensuring our geriatric population receives comprehensive, high-quality screenings and personalized prevention plans.
You will:
Specialized Clinical Focus AWVs provide patients with a no-cost, dedicated visit focused entirely on prevention and early detection. Patients receive a personalized care plan, medication review, and mental health screening, resulting in fewer hospitalizations and improved quality of life.
Autonomy with Support Travel to various sites independently with direct access to physician mentorship for complex case discussions and a collaborative team to handle follow-up care.
Flexible Professional Balance As a part-time role focused on AWVs, this position offers a predictable workflow without the burden of managing an acute primary care panel or high-intensity call schedules. It is ideal for a clinician seeking a high-impact role with a manageable pace
