Position: Wellness Nurse LPN
Pay: $34.00 per hour

Position Summary
The Wellness Nurse is responsible for promoting the health, safety, and well-being of residents in an assisted living community. This position provides nursing assessments, medication management, health monitoring, and coordination of care while ensuring compliance with state regulations and community policies. The Wellness Nurse works closely with residents, families, physicians, caregivers, and the leadership team to deliver compassionate, resident-centered care.
Essential Responsibilities
- Conduct resident health assessments and ongoing wellness evaluations.
- Administer medications and treatments as prescribed by physicians.
- Monitor residents for changes in health status and document findings accurately.
- Coordinate care with physicians, pharmacies, therapists, hospice providers, and other healthcare professionals.
- Respond promptly to resident emergencies and provide appropriate nursing interventions.
- Maintain accurate medical records and ensure compliance with state and federal regulations.
- Educate residents, families, and staff on health-related concerns and wellness initiatives.
- Assist with developing and updating individualized service plans.
- Supervise and support caregivers and medication technicians as needed.
- Participate in infection prevention and control practices.
- Promote a safe, respectful, and supportive environment for all residents.
- Participate in quality assurance and regulatory compliance activities.
Qualifications
- Current Licensed Practical Nurse (LPN) license in good standing.
- CPR and First Aid certification (or ability to obtain upon hire).
- Previous experience in assisted living, long-term care, geriatrics, or senior living preferred.
- Strong assessment, communication, and organizational skills.
- Ability to work independently and as part of a multidisciplinary team.
- Proficient in electronic health records and documentation.
