What you'll do

'/' The Opportunity

We are seeking a clinically accomplished, mission-driven physician leader to serve as Market Medical Director.

This is not a traditional administrative leadership role.

The Market Medical Director is responsible for building a high-performing clinical culture within their market by developing exceptional physicians, partnering closely with operations leaders, and ensuring Sage consistently delivers outstanding value-based care.

This role is equal parts physician leader, coach, educator, and practicing clinician.

You will oversee physician performance across multiple centers while maintaining a regular clinical presence to stay connected to the realities of patient care and the communities we serve.

Success in this role requires the ability to simultaneously develop people, improve systems, and deliver exceptional patient outcomes.

What Sets This Role Apart

Build Great Physicians

Your primary responsibility is developing exceptional PCPs. You will coach physicians in the art and science of value-based medicine and help them become more confident, capable, and fulfilled clinicians.

Lead Through Presence

This is not a remote oversight role. You will be visible in centers, present with teams, and engaged in day-to-day clinical operations.

Own Outcomes

You will be accountable for clinical quality, access, utilization, physician engagement, and patient experience.

Practice While You Lead

Unlike many physician leadership roles, you will remain actively engaged in patient care.

Build the Future

You will help recruit, onboard, retain, and develop the next generation of Sage physicians.


Core Responsibilities

Physician Talent Management, Development, & Clinical Leadership (40%)

  • Serve as the physician leader for PCPs across the market.
  • Conduct regular physician coaching sessions focused on clinical excellence, utilization management, and physician growth.
  • Lead Clinical Documentation & Diagnosis Check-Ins (CDDCs).
  • Develop individualized growth plans for physicians.
  • Support physician wellness and retention initiatives.
  • Partner with underperforming providers through formal coaching and performance improvement plans when necessary.
  • Foster a culture of accountability, learning, and collaboration.
  • Actively recruit, interview, and select top provider talent for Sage. 
  • Lead onboarding efforts ensuring new providers are effectively trained in value-based care and the Sage Health care model.
  • Build succession plans for future physician leaders in partnership with the CMO and VP, Head of People.
  • Proactively leads provider engagement, satisfaction, and retention efforts.

Value-Based Care Performance (25%)

Own market-level performance across:

  • Quality measures (HEDIS, STARS, CAHPS)
  • Risk adjustment and diagnosis recapture
  • Access and patient engagement
  • Referral optimization
  • Emergency department utilization
  • Hospital admissions
  • Medication management
  • End-of-life care quality

 Direct Patient Care & Clinical Support (15%)

The Market Medical Director will dedicate approximately a minimum of a half day per week to direct patient care activities that strengthen access, continuity, and support medically and socially complex patients throughout the market. 

This work will evolve based on market needs and may include:

New Patient Access

  • Conduct New Patient Appointments (NPAs) to improve access and accelerate panel growth during periods of high demand.
  • Support the onboarding of newly attributed patients into Sage's care model.
  • Model best practices in senior-focused primary care.

Continuity Support

  • Provide temporary clinical coverage during provider absences, onboarding periods, vacancies, and periods of rapid growth.
  • Preserve continuity and minimize patient disruption during provider transitions.
  • Partner with operations leaders to proactively identify and address access gaps.

Complex Care Support

  • Support PCPs caring for patients with advanced medical, social, and behavioral complexity.
  • Serve as an internal clinical resource for patients with frequent admissions, multiple specialists, diagnostic uncertainty, or escalating care needs.
  • Partner with PCPs to create comprehensive longitudinal care plans that help patients remain safely in their homes and communities.
  • Support goals-of-care discussions and transitions to palliative and hospice care when appropriate.

Build Institutional Clinical Memory

  • Identify patterns across complex cases and translate them into organizational learning.
  • Develop and disseminate best practices that elevate the capabilities of every PCP in the market.
  • Transform lessons learned from individual patients into systems that improve care for hundreds more.

The Market Medical Director does not replace the patient's PCP or maintain an independent patient panel. Rather, they function as a physician coach, clinical consultant, continuity partner, and force multiplier for the entire market.

Clinical Operations Dyad Partnership (15%)

Partner with market operations leaders to collaboratively focus on:

  • Improving patient access
  • Optimizing scheduling
  • Addressing staffing challenges
  • Standardizing workflows
  • Removing friction from physician and NP practice
  • Building, coaching, and developing highly functioning interdisciplinary teams to support the providers

Community Leadership (5%)

Serve as a visible ambassador for Sage by:

  • Building relationships with specialists and health systems
  • Engaging community organizations
  • Participating in payer relationships
  • Representing Sage in market initiatives


Qualifications

Required

  • MD or DO with active or eligible state licensure
  • Board certification/board eligibility in Internal Medicine, Family Medicine, Geriatrics, or Med-Peds (if board eligible, must demonstrate intention and plan to become certified within 6 months of hire).
  • 5+ years of clinical experience caring for medically complex adults
  • 3+ years of physician leadership experience with demonstrated success with leading high-performing teams.
  • Experience in Medicare Advantage, ACO, or other value-based care environments
  • Strong understanding of CMS quality programs, risk adjustment, and population health management
  • Excellent communication, coaching, and team-building capabilities

Preferred

  • Experience leading physicians across multiple sites
  • Experience in fully capitated Medicare Advantage models
  • Familiarity with HEDIS, STARS, CAHPS, RAF, and utilization management
  • Demonstrated success improving physician performance through coaching
  • Experience in integrating behavioral health and social determinants into primary care

  • No nights, no hospital call (outpatient primary care model) 
  • Sage Market Medical Directors can earn annual short-term bonus incentive compensation approaching 50% of salary through strong individual and market clinical quality, patient engagement, and value-based performance — with an additional long-term bonus incentive opportunity of 5–20% tied to sustained company financial excellence and tenure.
