Your role as Care Coordinator
Dialogue is looking for a Care Coordinator to join our team in order to improve the well-being of our members and meet the administrative needs of our healthcare team. In this hybrid role, you will focus on essential tasks such as processing prescriptions, sending referrals, scheduling follow-up appointments, coordinating with labs, following up with external clinics and supporting our members as needed.
