
3rd Street Youth Center & Clinic is a community-based agency providing youth in the Bayview Hunters Point with medical and behavioral health services that encourage them to make decisions that support their health, safety, and development. 3rd Street Youth Center & Clinic also manages a number of successful Transitional Age Youth (TAY) housing stabilization programs and one of the city's few Youth Access Points.
The Case Manager will provide comprehensive case management to TAY youth placed into housing.
Duties & Responsibilities:
The following is not designed to cover or contain a comprehensive listing of activities, duties or responsibilities that are required of the Housing Case Manager - Rising Up. Duties, responsibilities, and activities may change, or new ones may be assigned as needed.
● Individual case management & oversight to TAY referred from the Coordinated Entry System, including home visits and weekly check-ins
● Conduct intake and assessment; trauma screening, and provide housing navigation
● Resource brokerage and referral as needed
● Case coordination and advocacy within agency and with other CBO's
● Data collection and case notes entered into all appropriate databases.
● Support Housing Program Manager in all aspects of Rising Up (3rd Street's Rapid Rehousing program)
● Designing individual support & transition plan with youth including independent living skills and income stability.
● Attend community, city, & regional housing meetings
● Support for staff & youth in 3rd Street programming as needed

Education & Experience:
● Bachelors in related field OR equivalent work
● 2-3 years of experience working with high-risk and homeless youth.
● Experience in using the ONE/HMIS System preferred
● Working knowledge of issues facing youth.
● Valid California Driver's License and must also have daily access to a car.
● Experience working with youth experiencing homelessness and with diverse staff, clients, and volunteers.
● Experience with US Department of Housing and Urban Development (HUD) programs a plus
● Experience with San Francisco HSH policies a plus
● Proficient in Google Suite and Microsoft Office (preferred)
● Bilingual in Spanish (preferred)
General info:
● This is a full time & exempt position with an annual salary of $70,304 + benefits.
● This position works from our Housing Office in the Bayview/Hunters Point neighborhood.
● This position reports to the TAY RRH Program Manager.
