Home Care Nurse

McLean is a Life Plan community committed to caring for people and caring for nature.  We were founded thanks to the generosity and foresight of Senator George McLean over 55 years ago.  We have Independent Living, Assisted Living, Memory Care, and Skilled Nursing residents on our 110-acre campus.  We also house the McLean Center of Wellness, which includes Outpatient Rehabilitation.  We have a Homecare and Hospice Agency, and an Adult Day Program.  We steward over 4,400 acres of land with 28 miles of hiking trails at the McLean Game Refuge.

Per diem RN needed - Previous home care/hospice experience preferred

Pay Rate: $45.00 - $47.00/hr with mileage reimbursement

Hours: as needed, as available

Service area: - Farmington Valley and West Hartford areas.

As part of a person-centered care model, this role will provide skilled nursing care to clients in their homes.

Collaborate with an interdisciplinary team to achieve exceptional outcomes. 

Makes follow-up visit to provide skilled care to assess disease progress and evaluates response to treatment; provides support to patient/family utilizing teaching principles to promote and restore health and prevent disease and disability; coordinates care with providers.  Carries out the plan of care.  Teaches patients and reinforces client/caregiver teaching and education.  Refers, collaborates, and coordinates all client care services. Consults with physician regarding client needs and plan of care.

• Education: Graduate of an Associate’s Degree or diploma program in nursing 
• Experience: One year nursing experience or one year nursing experience in a specialty
• Licensure: Registered nurse with a license to practice in the state of CT
• Experience: one year home care experience
