What You’ll Do:

  • Provide in-office triage services Monday – Friday from 8:30 AM – 5 PM
  • Process patient referrals and facilitate new patient intake, including after-hours admissions when needed
  • Handle patient and family calls appropriately, including phone and in-home/facility visits when requested
  • Provide education, guidance, and emotional support to patients and families
  • Contact on-call visiting nurses to coordinate home or facility visits as needed
  • Document calls, visits, and clinical information accurately in the electronic medical record
  • Attend interdisciplinary team conferences, nursing meetings, and in-services as appropriate
  • Maintain necessary supplies and ensure readiness for patient care
  • Notify staff of patient deaths per protocol, ensuring compliance with Good Samaritan Hospice procedures

What We’re Looking For:

  • Clinical Expertise – Graduate of an accredited school of nursing with current Virginia LPN licensure
  • Experience – At least 2 years in acute care, home health, hospice, med/surg, or geriatric nursing
  • Strong Communication – Excellent verbal, written, and interpersonal skills; able to work collaboratively with team members
  • Independent & Reliable – Able to function without direct supervision while prioritizing patient needs
  • Tech-Savvy – Experience with electronic medical records and general computer skills
  • Professional & Dependable – Maintains confidentiality, demonstrates a positive attitude, and adheres to HIPAA and safety regulations

Qualifications:

  • Graduate from an accredited school of nursing
  • Current Virginia LPN license
  • Minimum 2 years’ relevant nursing experience
  • Experience with electronic medical records
