Position Summary

The Case Manager provides comprehensive case management services to participants across all Entrust programs. This role builds strong, supportive relationships with individuals and families, advocates on their behalf, and develops individualized service plans to reduce barriers and promote success. The Case Manager monitors progress, maintains detailed documentation, and collaborates with community partners and internal teams to ensure quality service delivery.


Key Responsibilities

  • Develop and implement individualized service plans for participants and families.
  • Conduct regular reassessments, updating plans and documentation as needed.
  • Maintain ongoing communication and supportive relationships with participants and partner agencies.
  • Collaborate with internal staff and external providers to ensure coordinated services.
  • Research and connect participants to community resources and supports.
  • Complete all documentation, reports, and participant updates accurately and on time.
  • Participate in meetings, trainings, and team collaborations.
  • Exit participants from programs according to established procedures.
  • Perform other duties as assigned to support program operations.

Qualifications

Education & Experience:
Any equivalent combination of education and experience that provides the required knowledge, skills, and abilities:

  • Bachelor’s degree in psychology, Sociology, Special Education, or related field and 1 year of experience; or
  • Associate’s degree in a related field and 2 years of experience; or
  • High school diploma/GED and 3 or more years of case management or human services experience, plus additional training as needed.

Required Skills & Knowledge:

  • Strong understanding of human services systems, community resources, and advocacy.
  • Ability to develop and implement service plans, reports, and participant goals.
  • Excellent communication skills—both written and verbal.
  • Proficiency in Microsoft Office (Word, Excel, Outlook, PowerPoint) and database systems such as SET-Works.
  • Ability to work collaboratively and maintain professionalism in sensitive or complex situations.

Preferred:

  • Current First Aid/CPR certification.
  • Experience in social services, case management, or related fields.

Additional Requirements

  • Valid Washington State driver’s license and proof of auto insurance.
  • Ability to pass a Washington State criminal background check, E-Verify, and mandatory drug screen.
  • Ability to perform essential job functions with or without reasonable accommodations.

Work Environment

  • Primarily office-based with occasional local travel.
  • Regular use of computer, phone, copier, and other office equipment.
  • Collaborative, supportive work culture with strong mission alignment.
