About the role
Entrust Community Services is seeking a dedicated and compassionate Case Manager to provide support, training, and advocacy for participants, enabling them to achieve independence and integration in their communities. The Case Manager will work collaboratively with individuals, families, and external partners to develop and implement individualized service plans (ISPs) tailored to each participant's needs.
What you'll do
Participant Support and Training:
- Provide one-on-one site training and support according to individual service plans.
- Orient new participants to their surroundings and teach self-management skills to promote independence.
- Offer transportation and mobility training to facilitate community navigation.
- Implement and recommend revisions to training plans to achieve desired goals.
- Assist participants with tasks until modifications can be made to support independence.
Advocacy and Community Education:
- Educate community members and non-paid supports on supervision, training, communication, and support needs.
- Support employers in modifying job descriptions, restructuring duties, or adjusting work environments to accommodate participant needs.
- Advocate for participant rights and strive to raise awareness of their capabilities.
Service Plan Implementation:
- Analyze environments to plan appropriate training, supervision, and support services.
- Conduct task analyses and identify learning barriers to determine effective training methods.
- Develop and implement schedules of support services, addressing behavior, emotional, hygiene, or personal barriers.
- Monitor and adjust service plans as necessary to stabilize or improve job performance.
Documentation and Reporting:
- Administer assessments, baselines, and evaluations while maintaining accurate records.
- Document progress through case notes, progress reports, and training logs.
- Communicate regularly with service teams and recommend changes to individual service plans as needed.
Team Collaboration:
- Participate in team planning sessions and ISP meetings to align on participant goals and needs.
- Maintain healthy relationships with internal staff, external agencies, care providers, and families.
- Facilitate the development and distribution of individual and family service plans.
MARGINAL DUTIES AND RESPONSIBILITIES
Advocacy and Tours:
- Conduct tours for stakeholders and community members as needed.
- Provide advocacy support services to protect participant rights.
Service Coordination:
- Intake and assessment processes.
- Perform periodic reassessments to update individual and family plans.
- Research community resources to support participants and make necessary referrals.
Sales and Employer Engagement:
- Develop and maintain employer relationships to expand opportunities for participants.
- Provide presentations about Entrust Community Services’ offerings to potential employers.
- Monitor employer databases and maintain progress reporting systems.
General Duties:
- Maintain accurate participant records in compliance with agency and accreditation standards.
- Ensure consistent communication with funding agencies, care providers, and families.
- Coordinate service coverage during team member absences.
- Perform additional duties as assigned to support the program’s mission.
Qualifications
- Strong interpersonal and communication skills.
- Ability to analyze environments and develop effective training methods.
- Proficiency in maintaining accurate records and reports.
- Knowledge of advocacy and community resources for individuals with disabilities.
- Flexibility to adjust plans and schedules to meet participant needs.
