About the role

The Case Manager provides comprehensive case management services to individuals and families across all programs offered by Entrust. This role involves developing individualized plans, monitoring progress, and collaborating with internal teams and external agencies to ensure participants receive the support they need.


Key Responsibilities:

  • Partner with Intake & Outreach staff during intake and assessment to develop and implement service plans.
  • Conduct periodic reassessments and maintain accurate documentation of participant progress.
  • Build and maintain supportive relationships with participants, families, and community partners.
  • Research and connect participants to community resources.
  • Communicate regularly with internal staff, external agencies, and care providers.
  • Ensure compliance with program rules and maintain accurate records in program databases.
  • Coordinate coverage during absences and perform other duties as assigned.

Qualifications:

Bachelor's degree in Psychology, Sociology, Special Education, or related field and 1 year of experience; OR Associate's degree and 2 years experience; OR High school diploma/GED with 3+ years of case management experience and additional human service training.

  • Strong knowledge of service planning, advocacy, and human service systems.
  • Excellent communication skills (oral and written).
  • Ability to develop plans, interpret technical reports, and meet deadlines.
  • Proficiency in Microsoft Office and database systems.

Requirements:

  • Valid Washington State driver’s license and proof of insurance.
  • Ability to pass background check, drug screen, and E-Verify.
  • Current First Aid/CPR certification preferred.

The pay range for this role is:

23.62 - 28.71 USD per hour (Walla Walla )
