Position Summary:
Entrust Community Services is seeking a dedicated Housing Placement Specialist to support individuals in securing and maintaining stable, independent housing. This role involves assessing client needs, developing individualized housing plans, providing housing-related support and education, collaborating with landlords and community agencies, and ensuring compliance with program requirements and documentation standards.


Key Responsibilities:

  • Conduct intake assessments to determine client housing needs and barriers.
  • Develop and implement Individualized Housing Plans (IHPs) that outline steps to secure and maintain housing.
  • Provide housing search assistance, including applications, showings, and advocacy.
  • Support clients with lease negotiations, understanding tenant rights, budgeting, and independent living skills.
  • Build and maintain relationships with landlords, property managers, and housing partners to identify housing opportunities.
  • Assist with crisis resolution, conflict mediation, and referrals to supportive services as needed.
  • Maintain accurate and timely documentation in case management systems and files.
  • Monitor progress and adjust housing plans to support long-term housing stability.

Qualifications:

  • High school diploma or GED required, degree in Social Work, Human Services, or related field preferred.
  • 2+ years of experience in housing support services, case management, or related areas.
  • Strong knowledge of local housing resources, tenant rights, and supportive housing models.
  • Effective communication, advocacy, and problem-solving skills.
  • Proficiency in Microsoft Office and case management software.
  • Valid driver’s license, proof of insurance, and ability to pass a background check and drug screening.

Physical and Work Environment:

  • Office-based with regular community and home visits.
  • Frequent travel required within service area.
  • Light lifting and active mobility may be required during housing tours or home setups.

Schedule:
Full-time, Monday–Friday, 8:00 AM–4:30 PM (some flexibility required based on client needs).

Performance Metrics:
Performance is evaluated based on housing placements secured, retention rates, client satisfaction, timely documentation, and community partnership development.
