Housing Specialist

As a Housing Specialist, you’ll support members in securing and maintaining safe, stable, and affordable housing. This includes housing placement, rental assistance navigation, and eviction prevention for individuals experiencing housing instability.

This role requires persistence, strong organization, and comfort working within complex housing systems while maintaining consistent in-office coordination.

What You’ll Do

  • Support members through all stages of the housing process, from assessment through placement
  • Assist members facing rental arrears or risk of eviction by coordinating housing stabilization and eviction prevention resources
  • Prepare, submit, and track housing applications and required documentation
  • Coordinate housing interviews and prepare members for the interview process
  • Communicate with housing providers, landlords, and community partners as appropriate
  • Track housing goals and progress within members’ Plans of Care
  • Build and maintain relationships with housing providers and community-based organizations
  • Serve as a housing resource for CareCollab staff
  • Document all housing-related services accurately in designated systems and EHRs
  • Participate in care planning meetings and interdisciplinary case conferences


What You Bring


Experience 

  • At least 1 year of experience in housing placement, housing navigation, eviction prevention, or supportive housing services including helping members navigate the legal housing system in NY, and accompanying them to court as needed
  • Experience working with rental assistance, HRA, housing stabilization, or eviction prevention programs preferred
  • Familiarity with NYC housing systems or subsidies strongly preferred

Skills

  • Strong organizational and documentation skills
  • Ability to manage multiple applications, deadlines, and follow-ups
  • Comfortable working both in-office and in the field
  • Strong communication skills and ability to advocate for members
  • Proficiency with Microsoft Office and care management systems
  • Bilingual abilities are a plus

Education

  • High School Diploma or equivalent required; Associate’s or Bachelor's degree preferred
