Case Manager

Crossroads serves adult women in two residential treatment programs, a women’s sober living facility, and men and women in one outpatient facility, all located in southern Maine. 


All of Crossroads’ clinical programs are evidence based and incorporate a trauma informed focus to support each client in their recovery journey. 

Crossroads, a highly respected nonprofit addiction and behavioral health treatment provider, is seeking a full time Case Manager to support Mothers and Children in our Residential substance use treatment program in Windham, Maine.

Our fully integrated treatment programs address the intricacies of how mental health and substance use disorders impact one another and allow women to explore the challenges they face and find what works best for their individual needs.

An ideal candidate demonstrates a strong understanding in the treatment of co-occurring disorders, family systems, and will possess strong relational qualities.

This position works closely with the Program Manager, clinical team and the medical team to ensure the day-to-day clinical care is aligned with agency policy and philosophy as well as state licensing and CARF standards.


Responsibilities include, but not limited to:

  • Provide case management services to individuals and families in need
  • Completes proper documentation according to agency and licensing standards utilizing the organizations electronic health record (EHR)
  • Conduct assessments to determine client needs and develop appropriate plans
  • Coordinate and monitor services provided by various agencies and organizations
  • Advocate for clients and ensure they have access to necessary resources and support
  • Provide crisis intervention and support during emergencies or difficult situations
  • Collaborate with other professionals, such as social workers and healthcare providers, to ensure comprehensive care for clients
  • Participates in agency, program and external meetings and committees as assigned
  • Represents, supports, and communicates Crossroads philosophy and mission
  • Attends residential program team meetings and provides support, feedback, and direction to the program staff
  • Models and supports the values and commitments of the organization
  • All other duties as assigned in support of organizational goals

Qualifications:
- Bachelor's degree in social work or a related field desired, not required
- Experience working with children, child protective services, or child welfare
- Knowledge of crisis management techniques and strategies
- Familiarity with behavioral health issues and interventions
- Strong communication and interpersonal skills
- Ability to effectively manage conflicts and resolve issues
- Experience in conducting assessments and developing service plans
- Ability to work independently and as part of a team
