Home Health Clinical Team Lead:

The Clinical Team Lead serves as a first-line clinical resource for the Home Health team, providing daily clinical oversight, chart review, workflow coordination, and clinical support to promote timely, accurate, compliant, and high-quality patient care. This role focuses on clinical documentation review, OASIS and Plan of Care review, orders management, Face-to-Face oversight, quality review, and centralized coordination of clinician scheduling, utilization, workload, productivity, and coverage.

This is a clinical leadership role without direct supervisory responsibility. The Clinical Team Lead provides guidance, feedback, coaching, and clinical support to staff while escalating unresolved, recurring, significant, or performance-related concerns to the Home Health Administrator.

What you will do as a Home Health Clinical Team Lead:

  • Perform daily chart review, including SOC, ROC, Recertification, Transfer, and Discharge documentation
  • Review OASIS, Plans of Care, orders, and Face-to-Face documentation for accuracy, completeness, and compliance
  • Identify documentation discrepancies and communicate needed corrections to clinicians
  • Oversee clinician schedules, workload, utilization, productivity, capacity, and coverage
  • Assist with visit redistribution and coverage planning for PTO, illness, and unexpected absences
  • Collaborate with Intake, Scheduling, Billing, clinicians, Administration, and the interdisciplinary team to resolve clinical and operational needs
  • Review validation and claims holds and follow up on missing or incomplete documentation
  • Participate in quality reviews and audits, identifying trends and opportunities for improvement
  • Provide clinical guidance, coaching, education, and support to Home Health clinicians
  • Serve as a first-line clinical resource while escalating significant or unresolved concerns to the Home Health Administrator
  • Maintain cross-training and provide coverage for the other Clinical Team Lead

What we are looking for in a Clinical Team Lead:

  • Current professional license as an RN, PT, OT, or ST and qualified to perform OASIS-related clinical review
  • Minimum of 1 year of Home Health experience
  • Strong clinical judgment, critical-thinking, organizational, and communication skills
  • Ability to review clinical documentation and identify inconsistencies or compliance concerns
  • Ability to provide constructive feedback and professional clinical guidance
  • Proficiency with electronic medical records and clinical documentation systems
  • OASIS and Medicare Home Health experience preferred
  • Experience with Plans of Care, orders, Face-to-Face documentation, or chart review preferred
  • Clinical auditing, quality, scheduling, utilization, or workflow experience preferred
  • MatrixCare or comparable Home Health EMR experience preferred
  • Previous preceptor, coordinator, clinical lead, or informal leadership experience preferred

Schedule, Pay, & Benefits:

Schedule: Monday–Friday, approximately 8:30 AM – 5:00 PM (with participation in on-call rotation as required)

Location: Roanoke and surrounding service areas

Starting Salary: 80,000/year

This is the starting salary for the position. Compensation may vary within the established salary range based on relevant experience, education, licensure, certifications, and other qualifications.
