Position Summary

The Associate Vice President (AVP) of Value-Based Care is responsible for enterprise-wide leadership, strategy, and execution of all value-based care (VBC) programs across IntraCare’s Medicare Shared Savings Program (MSSP) and Medicare Advantage (MA) portfolios.

This role holds direct accountability for Medical Expense Ratio (MER/MLR), risk adjustment performance, quality metrics, and value-based revenue generation. The AVP will partner closely with Clinical Leadership, Medical Directors, and Operations to design, implement, and continuously optimize programs that drive both clinical outcomes and financial performance.

This is a results-driven leadership role requiring a highly analytical, operationally engaged leader who can translate data into action and ensure execution at the provider and clinic level.


Reporting Structure

  • Reports to: Executive Leadership (CEO / CFO alignment)
  • Key partners: Medical Director(s), Clinic Operations, RCM, Analytics, Finance
  • Oversees: VBC team (coding, quality, care coordination, affiliate support)


Core Responsibilities

1. Enterprise VBC Strategy & Execution

  • Lead the design and execution of IntraCare’s value-based care strategy across MSSP and MA portfolios
  • Translate organizational growth goals into actionable clinical and operational workflows
  • Drive alignment between clinical, operational, and financial teams to achieve performance targets


2. Financial & Performance Accountability

  • Own performance across key VBC drivers:
    • MER / MLR performance
    • Risk adjustment capture and accuracy
    • Quality metrics and STAR ratings
    • Shared savings and incentive revenue
  • Develop and manage forecasts in partnership with Finance
  • Ensure performance is tracked at provider, clinic, and program level


3. 2026 Performance Ownership (Initial Mandate)

  • MSSP Savings Target: 10%
  • In-Year Quality Revenue: $2M
  • Risk Adjustment:
    • 95% recapture rate
    • Expansion of suspect condition capture
  • Quality Performance:
    • 4.25 STAR rating target


4. Future-State Performance (MA Expansion)

  • 2027 Targets:
    • 15% savings
    • 4.5 STAR rating
  • 2028 Targets:
    • 20% savings
    • 4.75 STAR rating
  • Build scalable infrastructure to support MA portfolio growth and increasing risk exposure


5. Clinical & Operational Integration

  • Partner with Medical Directors and Clinic Operations to:
    • Drive provider accountability for VBC metrics
    • Ensure compliance with care models (SAM, quality gap closure, RAF capture)
    • Align workflows with documentation and revenue capture requirements
  • Embed VBC performance into daily clinic operations (not siloed)


6. Risk Adjustment & Quality Optimization

  • Lead strategy and execution for:
    • RAF/HCC coding accuracy and capture
    • Suspect condition identification and closure
    • Quality gap closure workflows
  • Ensure real-time feedback loops to providers and staff


7. Data, Analytics & Reporting

  • Define and operationalize dashboards for:
    • MER / MLR tracking
    • Quality performance
    • Risk capture rates
    • Provider-level performance
  • Translate analytics into daily and weekly action plans


8. Program Development & Innovation

  • Design and implement new VBC programs and pilots
  • Evaluate and deploy technology solutions to enhance performance
  • Lead vendor evaluation, contracting, and oversight


9. Team Leadership & Development

  • Build and lead a high-performing VBC team
  • Establish clear accountability structures and performance metrics
  • Develop training programs for providers and staff on VBC workflows


10. External & Affiliate Alignment

  • Support alignment of affiliate providers with VBC goals
  • Collaborate with payer partners on performance improvement initiatives
  • Represent IntraCare in VBC strategy discussions and partnerships


Success Metrics

  • Achievement of MSSP and MA savings targets
  • Quality STAR rating improvement
  • Risk adjustment capture rates
  • VBC revenue growth (shared savings + quality incentives)
  • Provider compliance with VBC workflows
  • Reduction in cost of care and leakage


Qualifications

  • 10+ years of healthcare experience with progressive leadership responsibility
  • 5+ years in value-based care, population health, or managed care leadership
  • Proven accountability for:
    • MER/MLR performance
    • Risk adjustment programs
    • Quality outcomes
  • Experience in MSSP, Medicare Advantage, or similar risk-bearing models
  • Strong financial acumen with experience aligning clinical performance to revenue outcomes
  • Experience leading cross-functional teams in multi-site healthcare environments
