JOB PURPOSE  

As a Ve’ahavta Case Manager you will provide program participants with effective and personalized one-on-one case management as they work toward achieving their goals and creating positive change in their lives.


KEY ACCOUNTABILITIES   

In this role, you will be supporting individuals seeking guidance in navigating complex barriers to stability in their lives. These individuals are seeking supports in the areas of mental health, substance use, education, housing, conflict with the law, employment, immigration, and general direction in creating a plan to self-actualize their potential and move from instability to self-sufficiency.

 

Through consistent one-on-one case management meetings, and using a client-centered approach, you will assess their individual needs and develop action plans, with appropriate interventions and referrals, that will guide them towards achieving their life goals.

 

As part of the frontline team consisting of Program Coordinators, Case Managers, and Program Managers, as well as the VP of Programs, you will play a collaborative and instrumental role in developing our transformational programs for continued high impact outcomes and success.

 

*This is a hybrid position

 

QUALIFICATIONS/ REQUIRED SKILLS 

·       2-3 years of demonstrated knowledge and direct case management experience working with population as described

·       An undergraduate degree in Social Work, Psychology, or a related field strongly preferred

·       Existing knowledge of topics relating to barriers to socio-economic stability

·       Demonstrated experience in crisis intervention and group counselling/support services skills

·       Well-developed knowledge of an anti-oppression framework

·       Advocacy, networking, and resource development skills

·       Working knowledge of social service resources in the community

·       Excellent Microsoft Office Suite skills

·       Experience working with CRM and project management tools is required

·       Strong relationship building and communication skills

·       Passionate and values-driven

·       Exceptional work ethic and drive to make an impact

·       Superb professionalism, and desire to uphold a high standard of work

·       Team-oriented and collaborative

·       Excellent interpersonal skills, highly compassionate, patient, and supportive
