Nurse Practitioner - Lasalle County ($2,000 bonus)

The Role

This position is ideal for an experienced Nurse Practitioner who enjoys practicing independently while collaborating closely with physicians, facility staff, and interdisciplinary care teams. The NP will be responsible for managing a panel of medically complex geriatric patients, delivering high-quality care directly within the AL/IL and SNF settings, and helping improve patient outcomes while reducing avoidable hospitalizations.

Essential Responsibilities

  • Provide comprehensive primary and post-acute medical care to geriatric patients residing in assigned Skilled Nursing Facilities.
  • Collaborate with supervising physicians and interdisciplinary care teams to develop, implement, and evaluate individualized treatment plans.
  • Perform comprehensive patient assessments, routine follow-up visits, and acute care evaluations.
  • Diagnose, treat, and manage acute and chronic medical conditions commonly affecting older adults.
  • Perform preventative health screenings and facilitate advance care planning and goals-of-care discussions with patients and families.
  • Review, reconcile, and manage medication regimens to ensure accuracy, safety, and optimal therapeutic outcomes.
  • Order, interpret, and evaluate laboratory tests, diagnostic studies, and other clinical assessments.
  • Document patient encounters accurately and timely within the electronic medical record (EMR).
  • Coordinate care with nursing staff, therapists, specialists, hospital teams, and family members to ensure continuity of care across the healthcare continuum.
  • Respond to changes in patient condition and collaborate with facility staff to develop appropriate treatment plans that support quality outcomes and reduce unnecessary emergency department visits and hospital admissions.
  • Participate in quality improvement initiatives and ensure compliance with organizational policies and regulatory standards.
  • Travel daily between assigned AL/IL and SNFs located in Morris, Streator, Minooka, Sandwich, Dwight, Marseilles, and Plainfield.

Ideal Candidate

  • Board-certified NP
  • Interest in geriatrics (experience preferred but not required)
  • Comfortable in multiple care settings
  • Strong communicator with patients and families
  • Detail-oriented and thoughtful clinician
  • Values mentorship and team-based practice
  • New graduates with strong clinical rotations in geriatrics may be considered.
