Since then, Daybreak has expanded its mission to multiple locations adding both people and additional services. DBIS Serves hundreds of New Yorkers living with developmental disabilities.

Partnerships with local businesses like Stew Leonard, TJ Maxx, and Marshalls have allowed the vision to grow exponentially. In addition, taking on residential programs has allowed us to serve those that need round-the-clock supervision.

Job Title: Quality Assurance Investigator

Reports to: Quality Assurance Coordinator

Location: Multi-location (Bronx, Queens)

Job Classification: (Full-time)

FLSA: Non-exempt 

Salary: $30.00- $32.00 per hour


The primary mission of DBIS is to provide quality service to our individuals in a professional and caring manner. The Quality Assurance (QA) Investigator is responsible for conducting thorough, timely, and objective investigations into incidents, allegations, and regulatory concerns within the agency’s programs. This role ensures compliance with OPWDD standards, including 624 and 625 regulations, and supports the organization’s commitment to safety, transparency, and continuous improvement. The QA Investigator gathers and analyzes evidence, interviews involved parties, prepares clear investigative reports, and recommends corrective actions to strengthen program practices.

 

Education and Experience:

  • Bachelor's Degree in social work, psychology, human services, special education, or related field from a two-year college or equivalent
  • One year of direct experience working in Developmental Disabilities adult service programs (day and residential) preferred. (OPWDD) experience is preferred)
  • Previous investigative experience is a plus. Good verbal and writing skills
  • Minimum 2 years of experience with 14 NYCRR Parts 625 and 624 regulations (Incident Management & Abuse/Neglect Reporting).
  • Proficiency in Microsoft Office Suite, including Word, Excel, PowerPoint, and Access
  • Valid Driver’s License required for travel between program sites.


Summary of Primary Responsibilities and Essential Functions of Position:

  • Participates in in-service training and on-site workshops as required for professional growth and development. Attends external training and workshops at the direction of management.
  • Demonstrates a clear understanding of the purpose of investigations and the importance of the protection of the individuals served.
  • Adheres to best practices when conducting investigations as described in Investigator training.
  • Ensures that all aspects of assigned investigations are conducted, completed, and submitted per mandated deadlines according to NYS OPWDDs Part 624 and Part 625 regulations.
  • Provides feedback and follow-up as needed when requested by OPWDD, the Justice Center, and DBIS’s Incident Review Committee (IRC).
  •  Assists with the Annual Trends Analysis as directed by the CCO
  • Exhibits a good understanding of the various classifications of incidents and formulates investigatory questions based on these.
  • Prior to beginning an investigation, prepare a list of interview questions to be asked when obtaining testimonial evidence.
  • Gathers all relevant documentation needed to thoroughly investigate an incident, including, but not limited to,Life Plans, Staff Action Plans, Comprehensive Functional Assessments, Staff Schedules, Behavior Support Plans,ABC Sheets, 24-Hour Head Trauma Monitoring Sheets, Diet Plans, Clinic Visit Summaries, Urgent Care/Emergency Department Paperwork, Occurrence Reports, Body Check Sheets, Staff Training Documents, Law Enforcement Complaint Forms, Photographs, Physical Evidence, etc. Individuals' diagnoses, ability to communicate, the required level of supervision, and credibility should be included, as applicable.
  • Reviews past incidents and program-level occurrences dating back at least one year.
  • Conducts interviews in person unless there are extenuating circumstances that prohibit this. If someone must be interviewed over the telephone or cannot be interviewed, the reason for this must be explained in the investigative report.
  • Ensures that the instructions included on OPWDD 163s are followed when conducting abuse/neglect investigations.
  • For allegations of psychological abuse, ensure clinical assessments are provided as part of the investigations, as needed.
  • Provides completed investigation files pertaining to abuse/neglect to the Coordinator, Incident Management, for uploading into the WSIR within the required timeframe.
  • Uses positive approaches when handling difficult situations.
  • Communicates well in English and writes investigative reports that use language that is objective, clear, grammatically correct, and appropriate.
  • Practices good time management skills, the ability to prioritize tasks, organizational skills, and flexibility. Exhibits initiative and the ability to work independently. Ensures appropriate follow-up, as needed.
  • Excellent attention to detail
  • Ability to get along well with co-workers and many levels of administrative, management, and direct support staff

  • Capable of working under pressure and deadlines
  • Flexibility and a strong work ethic; willing to take on additional tasks and work beyond normal work hours when required
