The not-for-profit, 25-bed Critical Access Hospital features a 24-hour physician-staffed emergency department, with state-of-the-art diagnostic imaging and electronic medical record services. Our large, quiet labor/birthing rooms are private and located away from other patient care areas. The private patient rooms on the 5th floor Medical/Surgical Department boast views from every window.

About the role

The Clinical Coordinator is a multifunctional position with aspect of care management, risk assessment and quality assurance analysis and implementation. The Clinical Coordinator will be responsible for collaborating, facilitating and providing structure to ensure evidence based quality improvement, manages collection and analysis of clinical outcomes data in order to develop clinical process improvement initiatives, and be able to extract patterns from large sets of data in order to deliver business insights from data. Evaluate patient care data to ensure that care is provided in accordance with clinical guidelines and organizational standards, responds to and investigate any misconduct, noncompliance, or pattern of risk that is detected and collaborates with committee to develop and recommend a plan to cure the non-compliance or risk. Oversee post-discharge activities in order to ensure continuity of care as patient’s transition from inpatient to community. Identify patients that will benefit from case management. Provide ongoing education and support to medical providers and nursing staff to support improvement initiatives. Ensure that training and competency measurements are implemented for staff.


Qualifications

  • Licensed Registered Nurse or Licensed Practice Nurse.
  • Experience with EHR a must, as well as basic computer skills.
  • Knowledge of professional nursing theory and practice to give and evaluate patient care.
  • Knowledge of organizational policies, regulations and procedures to administer patient care.
  • Knowledge of medical equipment and instruments to administer patient care.
  • Knowledge of common safety hazards and precautions to establish a safe work environment.
  • Skill in applying and modifying the principles, methods and techniques of professional nursing to provide ongoing patient care.
  • Ability to assess the specific age-related needs of patients.
  • Skill in preparing and maintaining records, writing reports, and responding to correspondence.
  • Skill in establishing and maintaining effective working relationships with patients, medical staff and the public.
  • Ability to react calmly and effectively in emergency situations.
  • Experience in Care Coordination/Case Management as well as transitional care is beneficial.
  • Experience in leadership.

Starting pay range varies based on experience up to 15 years.
