What You'll Do

• Provide direct patient care under the supervision of a Registered Nurse (RN) or physician in accordance with care plans and facility policies.
• Administer medications and treatments as prescribed while monitoring residents for reactions or changes in condition.
• Monitor residents’ vital signs, health status, and overall well-being, reporting changes to the RN or physician.
• Assist with the development and implementation of patient care plans.
• Support and supervise Certified Nursing Assistants (CNAs) in providing daily care to residents.
• Maintain accurate and timely documentation of patient care, treatments, and observations.
• Ensure residents receive assistance with daily living activities and maintain comfort and dignity.
• Maintain a clean, safe, and supportive environment while following infection control and safety protocols.
• Communicate effectively with residents, families, and the healthcare team to support quality patient care.

Qualifications

• Active New Jersey Licensed Practical Nurse (LPN) license
• CPR/BLS Certification
• Fully COVID-19 vaccinated with booster (documentation required)
• Flu vaccination with documentation
• Valid state-issued ID or driver’s license
• Ability to work scheduled shifts (7:00am–3:00pm, 3:00pm–11:00pm, or 11:00pm–7:00am)
• Strong clinical and patient care skills
• Excellent communication and interpersonal skills
• Compassionate, dependable, and patient-focused attitude
• Ability to pass background screening and required employment verification
