What You’ll Do
As a Care Coordinator, you will oversee and manage the entire member journey from onboarding through diagnostics, plan development, care implementation, and renewal planning. You will ensure each step is personalized, accurate, and timely. You will serve as the primary point of contact for your assigned members, fostering trust through effective communication, scheduling, proactive outreach, and ongoing support. Your daily efforts have a significant impact on both clinical quality and emotional engagement.

You will collaborate with multiple teams to ensure care plans are executed and followed up on. You will track milestones, identify gaps, and promote teamwork across departments. From verifying diagnostic readiness to coordinating plan meetings and preparing mid-year reviews, you will ensure that every member’s experience reflects the Fountain Life standard of accuracy and excellence.

Key Responsibilities

  • Serve as the central point of contact for assigned members, guiding them through their year-long personalized care journey
  • Prepare members for diagnostics day: confirming intake forms, reviewing medical records, coordinating with internal teams, and scheduling appointments.
  • Attend Diagnostics Day and ensure a seamless, supportive member experience by coordinating transitions, checking timelines, and offering reassurance.
  • Maintain alignment across the care team by communicating updates, summarizing outcomes, and ensuring each member has shared visibility of their plan.
  • Schedule and prepare for post-diagnostic strategy meetings, mid-year recalibrations, and year-end reviews
  • Document milestones, flag outstanding tasks, and escalate potential delays or missed actions.
  • Communicate with members regularly to check on progress, answer questions, and offer support, especially when new interventions or updates occur
  • Collaborate with the Program Advisor and Care Team to prepare for renewal discussions with summaries and outcomes.
  • Provide white-glove, high-touch service that anticipates needs and removes friction at every stage of the member experience
  • Monitor and manage task checklists, follow-up cadence, and interdependencies across the team
  • Advocate for the member while reinforcing care team cohesion, accountability, and aligned delivery


Qualifications

  • Bachelor’s degree or equivalent experience in healthcare, health administration, concierge medicine, or wellness operations
  • Minimum of 2 years in a care coordination, member experience, or client service role
  • Experience working with electronic medical records (EMR), CRM systems, or healthcare scheduling tools
  • Outstanding organizational skills and the ability to multitask across clinical and administrative functions
  • Empathy, emotional intelligence, and professionalism in all interactions
  • Strong written and verbal communication skills
  • Commitment to delivering personalized service and supporting clinical excellence
