Karoo is seeking to add an important care team member, the Patient Engagement Specialist. This individual will be responsible for new patient acquisition and play a critical role in driving growth within the organization. This role will focus on screening eligible patients for enrollment, communicating with health plans, interacting with prospective patients to explain services, address inquiries, gather information, provide education, and schedule initial appointments with the virtual team. The PES will conduct outreach by phone and in person, educating patients about the benefits of the program, answering questions, addressing concerns, and guiding them through the enrollment process. This hybrid position includes 2-3 days of remote work each week and 2-3 days spent in the community, including hospitals, provider offices, and occasional patient home visits throughout the Chicagoland area.

Responsibilities

  • Contact eligible members by phone to introduce Karoo Health's cardiac care management program and conduct enrollment onboarding sessions.
  • Pre-screen patients for eligibility, educate patients about the benefits of participating in the program, and answer questions.
  • Build trusting relationships with patients and caregivers to encourage enrollment
  • Conduct in-person outreach in hospitals, provider offices, community settings, and patient homes as assigned.
  • Maintain documentation in the Karoo Health EHR system for tracking and reporting purposes 
  • Drive patient enrollment by providing an overview of the Karoo Health program and Model of Care through patient education and interaction to ensure patients understand program services and benefits
  • Maintain accurate documentation in the electronic care management platform.
  • Meet monthly outreach, enrollment, and engagement goals.
  • Manage inbound calls from patients responding to inquiries about enrollment and ongoing engagement with the Karoo Care Program
  • Implement strategies to appropriately enroll patients in the program 
  • Communicate regularly with all members of the interdisciplinary team to share key information, coordinate tasks and provide support for the enrollment process
  • Participate in team meetings, ongoing training, and quality improvement initiatives.
  • Special projects, initiatives, and other job duties as assigned 

Qualifications

  • High school graduate or equivalent
  • Ability to work within hours of 8 am-5 pm Monday-Friday
  • Community Health Worker/Partner, Medical Assistant, LPN, CNA preferred
  • Valid driver's license and proof of automobile insurance.
  • Cardiac-experience preferred 
  • Minimum three years of related experience in health care and/or physician clinic/practice setting 
  • General understanding of enrollment and billing processes and procedures
  • Strong interpersonal skills and ability to quickly establish rapport with providers and patients
  • Highly comfortable in group/team dynamics and able to fit in with clinic staff and teams
  • Confidence in explaining new programs and initiatives to people/patients; ability to connect and engage with people comfortably
  • Experience with team collaboration and coordination of care with external sources
  • Exceptional communication skills and can practice active listening
  • Strong written communication skills 
  • Comfort with ambiguity and a fast-paced culture; you can adapt and approach problems with a solution-oriented mindset 
  • Ability to utilize data to make informed decisions
