We’re looking for a Palliative Care Nurse Practitioner or Physician Assistant who is not only clinically skilled, but also compassionate, collaborative, and committed to improving quality of life for patients and their families.

We are looking for a team member in all service areas including the New River Valley, Roanoke, and Lynchburg area.

What You’ll Do:

  • Provide comprehensive palliative care services to patients in homes, facilities, and hospital settings
  • Conduct new patient visits, follow-ups, and ongoing patient management based on individual needs
  • Perform and document face-to-face encounters in accordance with Medicare and Medicaid requirements
  • Collaborate with the interdisciplinary team (IDT) to develop, implement, and evaluate individualized care plans
  • Provide expert guidance on pain and symptom management, prognosis, and treatment options
  • Educate and support patients and families on disease progression, care options, and goals of care
  • Coordinate care across settings to ensure continuity between home, hospital, and long-term care facilities
  • Facilitate family and care team meetings to align on patient goals and treatment plans
  • Prescribe medications and manage medical equipment needs in collaboration with the Medical Director
  • Maintain accurate, timely documentation in the electronic medical record
  • Participate in on-call rotation as needed, including evenings and weekends
  • Serve as a clinical resource for staff, providers, and community partners

  • Adaptable & Detail-Oriented – Able to manage changing priorities while maintaining accuracy and high-quality care

Qualifications:

  • Licensed Nurse Practitioner or Physician Assistant in the Commonwealth of Virginia required
  • Minimum of 2 years of experience in acute care, home health, hospice, palliative care, or geriatrics required
  • Prior palliative care and/or hospice experience preferred
  • Board certification in hospice and palliative medicine preferred
  • Valid driver’s license and insured automobile required
  • Strong clinical assessment, diagnostic, and documentation skills
  • Proficiency with electronic medical records and standard computer applications
  • Ability to maintain confidentiality and work with protected health information (PHI)
  • Commitment to the philosophy and principles of hospice and palliative care
