Role & Team Overview (Caregiver – Adult Foster Home) 

As a Caregiver in our Adult Foster Home (AFH), you will provide direct, hands-on support to residents in a small, home-like setting. This is a basic caregiving position focused on respectful, person-centered assistance with daily routines, safety, comfort, and quality of life. You will follow each resident’s care plan and house procedures, communicate clearly with teammates, and document observations and care provided as required. 

The Team You’ll Be Joining 

We are a Specialized Contract Home and maintain a consistent staffing model of two staff to five residents at all times. You will work side-by-side with another caregiver each shift, ensuring continuous coverage and the ability to respond quickly to resident needs. We place a strong emphasis on teamwork and a positive, respectful work environment—staff coordinate throughout the shift, share updates in real time, step in to help when a teammate is busy, and approach challenges with a “we’ve got this together” mindset. Our goal is a calm, supportive home for residents and a supportive team culture where everyone feels heard, backed up, and set up to succeed.

What you'll do

How the Role Works Day-to-Day 

• Team-based care: You and your shift partner communicate regularly about resident needs, priorities, and any changes in condition, and you actively support one another (assisting with lifts/transfers, covering brief breaks, and sharing tasks) to keep the shift running smoothly and safely. 

• Resident-centered routines: Support residents with activities of daily living (ADLs) such as bathing, dressing, grooming, toileting, mobility, and meals—based on individual preferences and care plans. 

• Safety and wellness monitoring: Observe residents for changes and report concerns promptly to the appropriate lead/supervisor and, when applicable, the delegated nurse or medical contact. 

• Medication management (as authorized): Assist with med tech tasks per delegation and house policy, including medication pass, MAR documentation, medication set-up/med box support, and tracking/ordering refills. 

• Documentation and communication: Record care, incidents, and key observations and communicate handoff notes to incoming staff. 

Qualifications

• Must be at least 18 years of age. 

• Must be able to pass a background check and any other screening required for work in an Adult Foster Home. 

• CPR training/certification required (or ability to obtain within a specified timeframe). 

• Ability to work well with others and maintain a respectful, team-focused attitude with residents, families, and coworkers. 

• Dependability is a priority—reliable attendance, punctuality, and follow-through on assigned tasks and documentation.
