Adult Nurse Practitioner (CA NP-103 License Required)

The Opportunity:

Pine Park Health is seeking an experienced, independently licensed Nurse Practitioner (California NP-103 license required) to join our growing team in the East Bay San Francisco market (Oakland, Walnut Creek, Concord).

This role is designed for a seasoned NP who is ready to practice at the top of their license — autonomously managing a panel of complex older adult patients within a value-based primary care model, without the constraints of a traditional clinic setting. If you're an experienced NP who wants clinical independence, a mission that matters, and the support of a motivated multidisciplinary team, this is your role.

What you'll do:

  • Provide onsite primary care services to older adults living in Senior Living Communities
  • Develop and execute detailed person-centered care plans on all primary care patients
  • Work with Medical Director to identify and develop policies to address issues that arise which affect day to day practice, culture or clinical guidelines, including for emerging disease(s)
  • Provide monthly Chronic Care Management (CCM) visits and care; review and sign off on CCM documentation
  • Run regular huddles and provide leadership to the clinical team
  • Identify ways of working with senior living management and care teams to better carry out Pine Park care plans and improve patient outcomes
  • Your clinical team includes dedicated RNs and Care Coordinators who handle the operational side — so you can stay focused on what you do best, caring for patients.

Qualifications:

  • Active California NP 103 Independent Practice License — required
  • AGNP strongly preferred. 
  • Minimum 5+ years of NP experience, with preference for geriatrics, primary care, or complex care management
  • Demonstrated ability to practice independently and manage a complex patient panel without direct physician oversight
  • Comfortable with a fast-paced, evolving care environment across multiple community sites
  • Strong clinical judgment with a genuine passion for caring for older adults
  • Collaborative leader who values the input of the full clinical team
  • Excellent written and verbal communication skills
  • Valid driver's license and willingness to travel within the East Bay San Francisco area
  • Comfortable with tech-enabled care delivery tools used to coordinate patient care and team function

The base salary range for this role is $177k–$187k total target (this includes base and performance bonus), equity, and benefits. Final compensation will reflect experience, depth of clinical background, licensure, and geographic market factors. These factors may include but are not limited to education, training, skill set, years and depth of experience, certifications and licensure, business needs, internal peer equity, and alignment with geographic and market data. Our compensation structures and ranges are tailored to each geographic zone's unique market conditions to ensure all employees receive fair and competitive compensation based on their roles and locations.
