Brain Health Navigator
About the Company
Sonnet Health is building a new way for people to understand, navigate, and act on their brain health. We're an early-stage company at the intersection of healthcare, technology, advances in neurological diagnostics and treatment, and care navigation.
We believe the time between a worry and an answer should be care, not a queue. Today, that time is spent waiting for a specialist, because all the work is bundled into one expert visit. We’re building a new model of care for that time, and a new vocabulary for a middle layer of medicine that has never had one.
About the role
We are looking for a Brain Health Navigator who loves guiding people through their journey, providing expertise at every step. You are a patient's guide, from their first call to their long-term plan. You'll support patients and their families across the full spectrum: people focused on prevention; people quietly concerned about health changes; people with clearly abnormal symptoms or test results; and people living with a diagnosis.
We're looking for an experienced navigator who brings real passion and expertise to this work. You'll work closely with our clinical team, including a board-certified neurologist.
You’ll join early enough to help shape our foundation. That means turning ideas into the workflows, systems, and tools behind a patient and family experience that feels genuinely delightful.
This is a fully remote role.
What you'll do
- Run intake calls with new potential patients to understand their goals and explain what comes next.
- Walk individuals and families through their Brain Health Report, and build a personal plan with them.
- Help coordinate tests and further evaluations ordered by our clinical team
- Provide ongoing check-ins that keep patients and families on track.
- Coordinate with patients' doctors, families, and caregivers, and connect them to helpful resources.
- Escalate concerns to our clinical team and keep clear records in our systems
- Help shape how navigation works at Sonnet as we grow.
What you bring
- 3+ years of experience in brain health, dementia, or related care, or in patient navigation.
- Solid knowledge of aging, cognition, mild cognitive impairment (MCI), dementia, and their risk factors.
- Genuine empathy, with the warmth and skill to handle hard conversations by phone and video.
- A background in nursing, social work, gerontology, or a related field; An RN or LCSW is a plus.
- A private, quiet workspace and reliable internet for confidential patient calls.
- Excitement about joining a fast-moving, early-stage startup, and comfort with ambiguity and evolving workflows.
Preferred / Bonus
- Navigator or dementia credentials, such as Certified Dementia Practitioner (CDP) or a recognized patient navigator certification.
- Familiarity with digital cognitive assessments and how to explain their results.
- A desire to share feedback and have a real voice in shaping how our product and workflows evolve.
Details
- Fully remote, open to candidates in all US states
- Part-time W-2 role, with a potential for full-time as we grow
- Compensation: $35-$55 per hour, based on licensure (such as RN or LCSW), certifications, and experience in dementia care and patient navigation. If your background doesn’t fit neatly into these categories, we’d still love to hear from you.
- Hours: 20-40 hours per week, with growth as our patient base expands. Most work happens during business hours in your patients’ time zones, with some evening and weekend calls to meet families when they’re available.
- Benefits: Shared during the interview process
