Job Description: HealthBar Home Care Nurse – HealthBar Home Care Program 

Job Title: Registered Nurse, Licensed Practical Nurse or Nurse Practitioner (see below) 

FLSA Exemption Status: Non-Exempt  

Organizational Alignment: Reports to the Concierge Coordinator 

Position Summary: As a Licensed Home Care Nurse, you’ll play a vital role in bringing exceptional care and comfort directly to patients’ homes. You’ll assess health needs in the home setting, provide medications and treatments in adherence to provider orders, and help patients and families feel confident managing care. You’ll monitor progress, coordinate with other providers, and ensure every visit supports safety, independence, and dignity. Your compassion, attention to detail, and commitment to patient well-being will make a lasting difference in the lives you touch. 

Essential Functions:  

  • Assess patient health status in the home environment  
  • Administer medications and treatments as prescribed in adherence to provider orders 
  • Monitor vital signs and symptoms to track progress or identify complications 
  • Educate patients and families on managing conditions, medications, and safety at home 
  • Coordinate care with physicians, therapists, and other providers 
  • Document care provided and update electronic health records (EHRs) accurately 
  • Ensure safety and infection control in the home setting 
  • Support patients’ independence and comfort in alignment with care goals 

 

Qualifications Required: 

  • Education: Graduation from an accredited nursing program (bachelor's or master’s degree is preferred.) 
  • Licensure: 
  • Active and unrestricted State of Michigan license as a Licensed Practical Nurse (LPN) or 
  • Active and unrestricted State of Michigan issued Registered Nurse License. or 
  • Active and unrestricted Family Nurse Practitioner (FNP) or Adult-Gerontology Nurse Practitioner (AGNP) National Board Certification from one of the following organizations: 
  • American Academy of Nurse Practitioners 
  • American Nurses Credentialing Center 
  • Active and unrestricted State of Michigan-issued Registered Nurse and Nurse Practitioner Specialty Certification License. 
  • Experience: Minimum of 2 years providing care as a nurse in applicable areas such as med-surg, ER, ICU, acute care, public health, school nursing or outpatient offices (preferred). 
  • Basic Life Support (BLS) Certification from the American Heart Association or American Red Cross within 90 days of employment. 
  • Additional Certifications: Preferred and highly encouraged from an accredited certification board, before or after employment in a specialty field such as: 
  • Health Coaching 
  • Case Management 
  • Primary Care 
  • Hospital Based (Med-surg, ICU, ED based certifications) 
  • Women's or Men's Health 
  • Sports Medicine 

 

Physical Requirements: 

This position requires the ability to provide hands-on patient care in the home environment. Duties may include standing, walking, bending, reaching, kneeling, and assisting patients with mobility. The role involves safely lifting, positioning, and transferring patients between bed, chair, and other locations, as well as providing support in the restroom and during other activities of daily living. Employees must use proper body mechanics and safety techniques at all times, and may be required to operate medical equipment. Visual and auditory acuity is necessary for patient assessment and communication.  In addition to hands-on patient care, the below physical requirements must be met: 

  • Ability to communicate verbally 
  • Fully functional vision is required (prescription lenses or contacts are acceptable) 
  • Prolonged periods of sitting, standing, or walking  
  • Ability to lift or carry up to 50 pounds occasionally  
  • Ability to operate a computer and other standard office or clinical tools  
  • Reliable transportation for occasional travel. The employee will not be responsible for transporting a client. 
  • Changing business conditions may trigger changes to physical requirements
