Position Overview:
HealthSnap (healthsnap.io) empowers patients and their physicians to improve health outcomes using an innovative platform for modern, proactive patient care. As pioneers of the next generation of healthcare, we are seeking engaging clinical leaders for our Nurse Supervisor positions to directly impact patient health through HealthSnap’s remote care solutions.
We are hiring Nurse Supervisors to manage a team of Care Navigators who conduct chronic care management and/or remote patient monitoring programs, in various states, in partnership with private practices and health systems. Successful candidates will bring supervisory experience and a high level of clinical acumen.
The role of a Nurse Supervisor is to create a team of engaged Care Navigators who are equipped to build trusting therapeutic relationships with their patients. This leads to improved patient outcomes, high Care Navigator retention and satisfied partners. Nurse Supervisors oversee patient-facing work and serve as the lead for performance and optimizing clinical outreach with selected practices. Above all else, you will play an essential role in cultivating highly engaged Care Navigator teams who simply love providing exceptional patient care in a remote setting.
Key Responsibilities:
Empowering Care Navigator and Patient success by developing supportive and trusting relationships with your team of 15 - 20 Care Navigators.
Equipping Care Navigators with the tools, skills, resources, and confidence to empower patients to manage their chronic conditions and achieve improved health outcomes.

Promoting and ensuring strict adherence to agreed-upon partner Work Flow Strategy Guides to support successful partnerships with our practices.
Developing and providing partner-specific workflow training to Care Navigators.
Closely collaborating with Clinical Operations Manager and Partner Success Team to deliver an outstanding partner experience.
Assigning patient caseloads and tasks to care navigators as needed.
Assisting with program enrollment and unenrollment as needed.
Monitoring and managing Care Navigator's daily performance.
Care Navigator coaching and shadowing on a regular and as-needed basis.
Conducting monthly one-on-one meetings with Care Navigators, more frequently as needed. Identify any help Care Navigator close any workflow gaps.
Completing monthly Quality Assurance reviews of Care Navigator calls and documentation. Provide any coaching, feedback, and ongoing monitoring based on Q/A findings.
Promoting strict adherence to all policies, procedures, legal and regulatory requirements.
Planning and leading weekly team meetings designed to share information, build relationships, and enhance CN engagement.
Additional duties as assigned.
Qualifications:
5+ years of experience in primary or specialty care practice, home care, or chronic care management.
Supervisor and/or Lead experience required, preferred in a remote setting.
LPN or greater certification
Experience in remote monitoring or virtual care management.
Track record of building highly motivated engaged teams, preferably in the remote setting.
High computer literacy, comfortable learning and using digital health portal and video calls
Strong communication and interpersonal skills
Possess excellent time management and organizational skills
Engaging personality with the desire to coach and mentor staff

The pay range for this role is:
58,000 - 62,000 USD per year (Remote - United States)
