Position Summary

The Care Coordination Support Specialist (CM Support) plays a vital role in helping our Case Managers implement each claimant's plan of care. This position focuses on coordinating appointments, transportation, diagnostic testing, therapy services, prescriptions, medical equipment, and other support services.

This is an excellent entry-level opportunity for someone with exceptional communication skills, strong attention to detail, and the ability to manage multiple tasks in a fast-paced environment.

Key Responsibilities

  • Schedule and confirm appointments with physicians, specialists, hospitals, therapy providers, and diagnostic facilities.
  • Coordinate transportation arrangements based on appointment details and client needs.
  • Assist with referrals, medical orders, prescriptions, therapies, medical supplies, and durable medical equipment (DME).
  • Communicate regularly with providers, facilities, pharmacies, transportation companies, and vendors via phone, email, fax, and secure messaging.
  • Research providers and vendors based on specialty, location, availability, and service requirements.
  • Monitor appointment schedules and manage cancellations, reschedules, and updates.
  • Follow up on outstanding tasks to ensure timely completion.
  • Escalate urgent or unresolved issues to the appropriate Case Manager.
  • Maintain accurate and timely documentation in the electronic health record (EHR) system.
  • Collaborate with clients, families, healthcare providers, attorneys, vendors, and the case management team to support continuity of care.

Qualifications

  • Excellent written and verbal English communication skills.
  • Professional and confident phone presence.
  • Strong organizational and multitasking abilities.
  • Ability to work independently and follow tasks through to completion.
  • Excellent attention to detail and problem-solving skills.
  • Proficiency with email, Microsoft Office, Google Workspace, and web-based platforms.
  • Reliable internet connection and a quiet home office environment.

Preferred Qualifications

  • Previous experience in customer service, administrative support, healthcare coordination, scheduling, or case management support.
  • Familiarity with medical terminology or healthcare environments.
  • Experience working with EHR/EMR systems.

Healthcare experience is preferred but not required. We welcome candidates with strong communication skills, excellent organization, and a willingness to learn.
