About the role

The home health aide provides personal care and related services in patients' homes. They function under the direction, instruction, and supervision of the nursing supervisor, director, and/or CNO. 

What you'll do

  • Assist with ADLS and iADLs as per the patient's plan of care (POC).
  • Follow the POC for each patient and document the same.
  • Help patients to maintain good personal hygiene.
  • Meal preparation as per the plan of care
  • Assist patients with ambulation as per the POC.
  • Assist with certain treatments as directed by the Skilled nurse.
  • Assist therapy personnel as needed with rehabilitative processes per the HHA POC
  • May cue for medications ordinarily self-administered as assigned.
  • Taking and charting vital signs under the direction of a nurse or healthcare practitioner
  • Attempts to promote patient’s mental alertness through involvement in activities.
  • Gives simple emotional support to patients and other members of the household.
  • Prepares a report of visits on the day they are performed and incorporates it into the clinical record weekly or as directed.
  • Report any changes in the patient’s status or home situation immediately to the supervisor, staff nurse, or aide supervisor, including recognizing & reporting skin changes.
  • Conduct assignments as instructed by the supervising nurse or therapist, and document and report when unable to do so.
  • Serve as an active participant on the patient's multidisciplinary care team, attending case conferences as required.
  • Work with personnel of other community agencies involved in the patient’s care as directed.
  • As instructed by the nurse, perform routine housekeeping tasks related to a safe and comfortable environment for the patient.
  • Attend in-service as required by state and federal regulations.
  • Confirm the schedule of visits weekly.
  • Actively participate in the company's QA program.
  • Other duties as assigned.

Qualifications

  • GED or High school diploma, preferred. 
  • Ability to read and write in English. 
  • Experience with home health services within the previous 24 months. 
  • Registration with the TX Department of Human Services (DHS’s) NAR with no findings of client abuse, neglect, or misappropriation of client’s property. 
  • Must pass a criminal background check and have current CPR certification. 
  • Meets one of the following education requirements:
    • Satisfactorily completed a training & competency evaluation program that complies with the requirements of TX Code 558.701 and CMS regulation (minimum 75 hours) and have a certificate of completion; or,
    • Satisfactorily completed a competency evaluation program that complies with the TX Code 558.701 and CMS regulation; or,
    • Having documentation from the director of programs or the dean of a school of nursing that states the candidate is a nursing student who has demonstrated competency in providing basic nursing skills in accordance with the school's curriculum; or,
    • Listed on the HHSC nurse aide registry (NAR) with no finding against the aide relating to client abuse or neglect or misappropriation of client property; or,
    • Meets the requirements of a TX state licensure program that meets the provisions set forth by CMS Conditions of Participation for Home Health Agencies for Home Health Aide training & competency. 
  • Has the following work or volunteer experience:
    • Minimum of one-year full-time experience in direct client care in an institutional setting (hospital or nursing facility); or,
    • One year of full-time experience within the last five (5) years in direct client care in an agency setting.

The pay range for this role is:

12 - 20 USD per hour (DFW)
