About the role

A Licensed Vocational Nurse provides practical nursing skills within the scope of nursing practice standards in collaboration with RN case managers/supervisors and teaches and educates clients and their families according to the established professional plan of care. Licensed Practical nursing is performed under the supervision of the assigned RNs, Administrator, and Supervising Nurse.  

Note: This position pertains to in-home client services and telehealth in the DFW, Texas.

What you'll do

  • Maintain the nursing frequency established by the supervising RN.

  •  Follow the plan of care established by the supervising RN. 

  • Reporting to the RN.

  • Prepare equipment & materials for treatments, observing aseptic techniques as required. 

  • Perform home safety checks and environmental checks of the client’s home environment on all visits.

  • Report any unsafe or potentially unsafe condition to the supervising RN. 

  • Perform nursing visits and documents accordingly. 

  • Review and follow the supervising RN’s plan of care, communicating frequently with the supervising RN about any necessary changes. 

  • Collaborate with the supervising RN weekly (or more frequently) to benchmark toward the established outcome goals. 

  • Review the client’s ADL and iADL abilities.

  •  Teach clients & family self-care techniques.

  •  Communicate any suggested changes to the supervising RN.

  •  Reinforce the HHA plan of care when HHA services are ordered.

  • Effectively communicate with client/family the POC and progression of such. Keep the client informed ongoing.

  • Effectively communicate with other disciplines in the case (actively participates in case conferencing) to effectively and appropriately problem-solve as situations arise.

  • Communicate effectively with the supervising RN, plan scheduled visits, and make schedule changes weekly, as needed. Although the clinician's caseload is self-scheduled, communication of the clinician’s schedule to the nursing supervisor is essential.

  • Participate in case conferencing sessions. Effectively functioning as an involved team member to collaborate on best practices for the individual client’s needs.

  •  Submit accurate, complete paperwork at the end of every week so that all medical records are intact and up to date. Uses the drop box if the office is already closed for the weekend.

  •  Notify the supervising RN with requests for re-authorization of “more visits needed” prior to the third-party insurers' authorized number of visits expiring.

  •  Suggest community services that may be available to the client to assist with safe home care needs.

  •  Participate in the Performance Improvement Committee process as requested.

  •  Participate in staff meetings. Adjusts visits on the staff meeting day to accommodate this function.

  •  Participate in the growth of the office by being a willing preceptor for newly hired same-discipline employees.

  •  Maintain LVN licensure and actively seek out educational experiences to enhance their practice of home care nursing and the benefit of the clients they serve.

  •  Notify the Agency of emergencies, sickness, and other imminent occurrences that may affect the client caseload as quickly as possible relative to the event’s occurrence.

  •  Submit written time requests 2 weeks or more in advance of planned time off.

  •  Other duties as assigned. 

Qualifications

  • Graduate of an approved LVN nursing school.

  • Active license to practice professional practical/vocational nursing in Texas.

  • Minimum one (1) year of experience as an LVN, preferably in a home care setting. 

  • Able to pass a criminal background check and any other required checks

  • Current CPR certification.
