Job Summary:
IntraCare is seeking a Certified Diabetes Care & Education Specialist (CDCES) who is also a Registered Dietitian (RD/RDN) to deliver integrated, expert care for patients with diabetes. This role blends Diabetes Self-Management Education and Support (DSMES) with personalized Medical Nutrition Therapy (MNT), focusing on prevention, comorbidity management, and empowering patients to take control of their health. The ideal candidate will be aligned with IntraCare’s mission to provide value-based, patient-centered care through collaboration and innovation.
Key Responsibilities:
1. Patient Care & Education
- Conduct individualized assessments to create personalized diabetes education and nutrition plans.
- Deliver DSMES using evidence-based frameworks (e.g., AADE7), covering topics such as blood glucose monitoring, insulin usage, medication adherence, and lifestyle changes.
- Provide tailored nutrition therapy addressing glycemic control, weight management, and related comorbidities such as hypertension and cardiovascular disease.
2. Care Team Collaboration
- Collaborate closely with primary care providers, endocrinologists, and interdisciplinary teams to optimize patient outcomes—especially for high-risk populations.
- Participate in case reviews and coordinate with Remote Patient Monitoring (RPM) teams and community partners for continuity of care.
3. Program Development & Support
- Develop and facilitate diabetes education workshops for both patients and internal staff.
- Create and support the development of educational content for telehealth and RPM platforms.
4. Documentation & Quality Assurance
- Accurately document all patient interactions in the electronic health record (EHR), ensuring compliance with HIPAA and internal protocols.
- Track clinical outcomes (e.g., A1C, BMI, blood pressure) and adapt interventions based on patient progress.
5. Community Engagement
- Represent IntraCare at community events, health fairs, and screenings to raise awareness and promote diabetes prevention and education initiatives.
Qualifications:
Education:
- Bachelor’s degree in Nutrition or Dietetics required; Master’s degree preferred.
Certifications:
- Registered Dietitian (RD/RDN) with active licensure in Texas and/or Arizona (as applicable).
- Certified Diabetes Care & Education Specialist (CDCES) – required.
- Certified Insulin Pump Trainer – preferred.
Experience:
- Minimum of 2 years of clinical experience in diabetes education and nutrition counseling.
- Familiarity with Electronic Health Records (EHR) and the Nutrition Care Process (NCP).
Skills:
- Excellent interpersonal and communication skills, with the ability to engage diverse patient populations.
- Strong organizational skills and ability to work independently and collaboratively within a multidisciplinary team.
- Proficiency in Microsoft Office tools; experience with telehealth platforms and patient education technology is a plus.
