The Role

We are looking for a Cardiac Care Coordinator to join our clinical team. This role is ideal for a Certified Medical Assistant with a background in cardiology who thrives in a fast-moving environment and enjoys supporting both patients and care teams.

The Care Coordinator will work hand-in-hand with RNs, LCSWs, and other members of the Karoo care team to ensure patients get timely, coordinated, and seamless care. As Karoo grows, this role will expand with new responsibilities and opportunities to shape how we deliver patient-centered support.

What You'll Do

  • Provide day-to-day support to the clinical team to ensure patients receive timely, coordinated care.
  • Provide care coordination and direct assistance to low-complexity patients, including outreach, follow-up, and navigation of care. This often involves “cold calls” or unexpected calls to patients, in which patience (and empathy) and repetitive efforts are often required.
  • Assist with patient scheduling, enrollment, and engagement activities.
  • Help review, organize, and route patient data and documentation to the appropriate team members.
  • Support care plan preparation, distribution, and compliance requirements.
  • Offer light technology and workflow support to the clinical team.
  • Capture clinical meeting notes, track follow-up items, and ensure action items are completed. 
  • Assist with onboarding and orientation of new care team members.
  • Handle medical records requests and other administrative tasks.
  • Participate in member engagement efforts, including calls, texts, and email campaigns. 
  • In the event of a Clinical Team member’s absence/illness or scheduling conflicts, call the patients to reschedule appointments as requested.
  • Take on additional duties as the role evolves with the organization.

What We're Looking For

  • Certified Medical Assistant (required).
  • Fluent Spanish speaker (required).
  • Experience in a cardiology practice strongly preferred.
  • Prior experience supporting interdisciplinary clinical teams, telehealth experience a strong plus. Experience recording and tracking details, action items, and reporting concerns.
  • Comfortable with direct patient communication by phone, text, and email. 
  • Strong organizational skills with the ability to manage multiple tasks in a fast-changing environment.  Tools:  Google suite, and other tracking software proficiency required.
  • Tech-savvy and eager to learn new systems quickly.
  • Passion for improving patient outcomes and supporting care teams.
  • Comfort working in a fast-paced, early-stage startup environment, where priorities shift and structure is still evolving.
