Associate Care Manager

About the Role


Job Title: Associate Care Manager
Location: Newton/Metrowest
Job Type: Part-time (25 hours per week, Monday through Friday)


Job Summary:


The Associate Care Manager will support the Care Management team in providing care to their clients.  This may include, but is not limited to: accompanying clients to medical appointments, accompanying clients to or providing shopping and other errands, assisting Care Managers with research and resource gathering. Communication with the Life Care Manager will be required for seamless implementation of the support need.  Professional, clear communication with family and/or other collaterals and decision-makers may also be required.


Responsibilities:

  • Assist Life Care Managers in supporting clients by accompanying or meeting clients at medical appointments; providing companionship; assisting with errands and shopping. 
  • Effectively communicate and collaborate with Life Care Manager regarding support needs, follow up, communication with collaterals.
  • Develop professional, effective relationships with clients, families, responsible parties, and care providers in all care settings.
  • Provide client-centered problem-solving, ongoing support, and advocacy for clients as needed. 
  • Provide support and education to family/loved ones coping with loss, dementia, illness, change, resistance.
  • Document billing and interventions daily
  • Attend weekly in-person supervision with supervisor
  • Attend monthly Group Supervision
  • Attend scheduled Staff Meetings, facility tours
  • Abide by Standards of Practice for Aging Life Care Professionals as determined by ALCA.


Qualifications:

  • Undergraduate degree and/or licensure in social work, nursing, geriatrics, mental health, or related field
  • 2-4 years’ experience working with older adults and families
  • Excellent clinical skills
  • Ability to drive, access to car
  • Highly organized, ability to work in non-traditional, highly professional work environment, and manage caseload effectively
  • Outstanding interpersonal and communication skills

In accordance with the Massachusetts Pay Transparency Act, the expected pay range for this position is $30–$35 hourly. Actual compensation may vary based on factors such as licensure, years of experience, and other relevant qualifications.
