Job Title: Case Manager – Virtual Intensive Outpatient Program (IOP)
Location: Remote
Employment Type: Part-Time (10 hours per week)

 Reports To: IOP Manager



Position Summary

We are seeking a compassionate, organized, and detail-oriented Case Manager to support clients enrolled in our Virtual IOP. The Case Manager will work collaboratively with the IOP team to ensure clients receive individualized support, continuity of care, and access to necessary resources. This position requires excellent communication skills, a strong understanding of addiction and mental health systems, and the ability to operate effectively in a remote, fast-paced setting.




Key Responsibilities

  • Facilitate screenings for interested individuals to determine appropriate fit for a virtual IOP experience.
  • Conduct biopsychosocial assessments and develop individualized recovery plans.
  • Coordinate with therapists, group facilitators, and medical providers to support integrated care.
  • Provide referrals to housing, employment assistance, legal services, educational resources, and community support.
  • Monitor and document client progress, engagement, and progress towards treatment goals.
  • Maintain timely and accurate documentation in Meridian’s identified electronic medical records platform.
  • Participate in multidisciplinary team meetings and collaborate on treatment/recovery planning.
  • Provide crisis intervention and support as needed, escalating appropriately.
  • Advocate for client needs and assist in removing barriers to recovery.
  • Ensure discharge planning is initiated early and includes comprehensive transition and aftercare coordination.
  • Stay informed on community resources, insurance coverage requirements, and evolving best practices in behavioral health.




Schedule & Compensation

  • Part-time schedule with availability Tuesday, Wednesday, Thursday from 4-7pm to accommodate client schedules, IOP scheduling and or to address concerns as they arise. (About 10 hours per week)
  • Compensation $20-23 per hour depending on experience
