About the role
The Nurse Practitioner (NP) provides comprehensive primary care within a federally qualified health center (FQHC) serving youth in a congregate care and crisis shelter setting. Care includes preventive services, acute and chronic disease management, and interdisciplinary coordination using a trauma-informed, harm-reduction approach. The NP is responsible for assessment, diagnosis, and treatment, and collaborates with clinical and program teams to support continuity of care and population health outcomes.
What you'll do
CORE RESPONSIBILITIES – CLINICAL CARE
· Perform comprehensive medical evaluations including history, physical exams, and risk assessments
· Provide primary and preventive care including screening, vaccination, and health maintenance
· Diagnose and manage acute and chronic conditions
· Prescribe and manage medications, including controlled substances, in accordance with NYS regulations
· Provide gender-affirming care, sexual/reproductive health services, and HIV/PrEP management
· Conduct clinical workups including labs, imaging, and specialty referrals
· In the event of a crisis on a residential floor or involving a patient, the provider is responsible for delivering care and support within the limits of their scope of practice and ensuring prompt escalation to appropriate clinical personnel or emergency services when warranted.
MEDICATION MANAGEMENT
· Initiate, prescribe, and manage medication regimens in accordance with NYS regulations and clinical guidelines
· Monitor treatment response, adherence, side effects, and clinical outcomes, and adjust therapy as indicated
· Complete prior authorizations and support timely access to medications to ensure continuity of care
· Comply with all controlled substance regulations, including iSTOP (PMP) requirements
· Provide patient education on medication use, adherence, side effects, interactions, and harm reduction strategies
CARE COORDINATION
· Collaborate with interdisciplinary teams including medical, behavioral health, and program leadership
· Coordinate care within a congregate care and crisis shelter environment, ensuring continuity across services
· Facilitate referrals, diagnostic workups, and follow-up for specialty care
· Identify and manage clinical and psychosocial risk factors
· Participate in interdisciplinary case discussions and care planning, including daily huddles and red flag meetings
· Communicate clinical plans and updates to team members to support timely and informed decision-making
· Support care transitions and coordination with external providers and healthcare systems
DOCUMENTATION & SYSTEMS
· Document encounters in appropriate EHRs (including eClinicalWorks and ETO) in a timely and compliant manner
· Manage orders, referrals, and results within the EHR
· Complete required disease reporting (e.g., PRISM)
· Ensure compliance with HIPAA and required training standards
POPULATION HEALTH
· Support population health initiatives and identify high-risk patients
· Participate in quality improvement and FQHC/HRSA standards, including risk management and continuous improvement
· Provide patient education and promote harm reduction strategies
Qualifications
· Master’s degree or higher in Nursing
· Licensed Nurse Practitioner/RN in New York State
· National certification required
· 1+ year NP experience preferred
· Experience in FQHC, primary care, or underserved populations preferred
SKILLS
· Strong clinical decision-making and diagnostic skills
· Experience with EHR systems (eClinicalWorks preferred
