As a Home Health Licensed Practical Nurse provides skilled nursing services to patients in the comfort of their own homes, helping patients maintain their independence, safety, and quality of life. This role offers the opportunity to build meaningful relationships with patients and families while providing personalized, one-on-one care. As an LPN you work as an important part of the care team, providing skilled nursing interventions, education, and support based on each patient's individual needs. You will work closely with patients, families, physicians, Registered Nurses, and other healthcare professionals to provide coordinated, compassionate care in accordance with physician orders and the established Plan of Care.

What you will do as a Home Health LPN:

  • Provide skilled nursing visits to patients in their homes
  • Perform patient visits in a timely, professional, and appropriate manner according to standards of care
  • Provide skilled nursing care as assigned by the Registered Nurse and according to physician orders
  • Monitor patient conditions, recognize changes or symptoms, and report concerns appropriately
  • Coordinate patient care with physicians, nurses, office staff, and other healthcare providers
  • Contact physicians regarding changes in patient condition and receive and document verbal orders
  • Educate patients and families on prescribed activities designed to improve independence and functionality
  • Review and update medication profiles while assessing medication compliance, understanding, contraindications, and side effects
  • Ensure laboratory tests are appropriately obtained, stored, and transported
  • Ensure missed visits are appropriately rescheduled and the patient's Plan of Care is followed
  • Participate in weekly case conferences, staff meetings, in-services, and continuing education
  • Participate in an on-call rotation to provide after-hours and weekend evaluations and visits as directed

What we are looking for in a Home Health LPN:

  • Graduate of an approved nursing program
  • At least 1 year of experience as a licensed professional nurse
  • Current Virginia nursing license or compact state license
  • Medicare/OASIS Home Health or Hospice experience preferred
  • Current BLS/CPR certification or obtained within 30 days of hire
  • Strong communication, decision-making, and critical thinking skills
  • Comfortable using EMR systems and technology
  • Current driver's license and reliable form of transportation

Schedule & Benefits:

Schedule: Full-time, Monday–Friday, with participation in an on-call rotation as required

Location: Roanoke and surrounding service areas
