Position Summary: 

The Director of Real Estate & Revenue Strategy is the senior leader responsible for MDpanel's entire physical site portfolio across California: 70+ flagship evaluation centers under direct lease and 230+ per diem sub-lease locations. This is a strategic leadership role with multi-million dollars in annual real estate budget authority.

This role operates at the intersection of real estate strategy and revenue growth. The Director of Real Estate & Revenue Strategy leads strategically while empower their team to directly manage day-to-day facility maintenance or per diem site operations. This role focuses on flagship site expansion into high-demand zip codes, strategic portfolio consolidation to drive EBITDA improvement, site-level ROI modeling with FP&A, physician experience as a retention lever, and geographic market development that directly enables physician panel growth and evaluation revenue.

Reporting to the SVP & GM, this role manages a small team and partners cross-functionally with Finance/FP&A, Legal, Growth, Scheduling, Customer Success, and Technology teams. The ideal candidate combines commercial real estate expertise with a revenue-minded operating approach, using data to connect site decisions to physician retention, evaluation volume, and financial performance.


Essential Duties and Responsibilities:  

Flagship Site Strategy & Geographic Expansion

  • Own MDpanel's flagship site expansion strategy, using DWC paneling data, zip-code-level evaluation volume, specialty demand (orthopedic, neurology, pain medicine, psychiatry, etc), and physician panel density to identify and prioritize new flagship locations.
  • Lead the full site acquisition lifecycle for new flagships: market analysis, site identification, LOI negotiation, lease execution, build-out oversight, and go-live activation, targeting 6-10 new flagship openings annually in top-paneled zip codes.
  • Develop and maintain a rolling 12-month site expansion roadmap aligned with the Growth team's physician recruitment pipeline and the company's revenue targets.
  • Evaluate emerging market opportunities across California and potential national expansion, building business cases that quantify projected evaluation volume, revenue contribution, and payback period for each site investment.
  • Monitor competitor site footprints (ExamWorks, Proficient, Arrowhead, Guardian, etc) and secure premium locations in high-demand markets before competitors establish presence.

Strategic Portfolio Consolidation & EBITDA Optimization

  • Drive ongoing portfolio rationalization to maximize EBITDA contribution: identify underperforming sites for renegotiation, downsizing, consolidation, or exit.
  • Lead all major lease negotiations and renewals across the flagship portfolio, driving favorable economics on base rent, tenant improvement allowances, escalation caps, renewal options, and early termination flexibility.
  • Build and own a site-level ROI model in partnership with FP&A, ensuring every major lease renewal, termination, and new site decision is backed by clear financial rationale with revenue attribution.
  • Deliver measurable annual cost savings targets through lease renegotiation, space optimization, and vendor consolidation, with full transparency into occupancy cost trends for leadership.

Site Infrastructure, Security & Technology Systems

  • Own the strategic direction of MDpanel's site access and security infrastructure and centralize inventory controls across flagships and applicable per diem locations.
  • Drive ongoing Flagship improvement: standardized exam tables, equipment, furniture, amenities, signage, and brand presentation across all flagship offices to ensure consistent physician and applicant experience.
  • Evaluate and implement technology solutions that improve site operations: smart access systems, remote monitoring, digital signage, automated work order tracking, and centralized asset management.
  • Establish and enforce facility standards and SOPs covering the full lease lifecycle, key/inventory management, site activation protocols, and brand standards.

Physician Experience & Retention Through Real Estate

  • Position real estate as a measurable physician retention and satisfaction driver, targeting less than 5% of evaluation days impacted by access, facility, or equipment issues across the entire network.
  • Drive improvement in physician and applicant facility-experience satisfaction scores through site upgrades, equipment standardization, and operational reliability.
  • Align site strategy with physician preferences and specialty requirements, ensuring flagships in high-value specialties have the appropriate exam room configurations, equipment, and amenities.

Per Diem Portfolio Strategy & Oversight

  • Set the strategic direction and conduct annual review for the 230+ per diem sub-lease portfolio: site selection/replacement criteria, pricing parameters, vendor qualification standards, and geographic coverage requirements.
  • Oversee the Real Estate Operations Administrator who manages day-to-day per diem site relationships, quality audits, vendor scheduling, and maintenance coordination.
  • Negotiate and structure new per diem site commitments as needed with full security/access infrastructure, targeting 12+ new per diem agreements annually in markets that complement the flagship footprint.

Insurance, Risk Management & Compliance

  • Manage property insurance and general liability coverage across the entire site portfolio, coordinating with brokers and carriers on policy renewals, claims management, and coverage optimization.
  • Ensure all flagship and per diem sites maintain compliance with DWC paneling requirements, ADA accessibility standards, OSHA workplace safety, infection control protocols, and healthcare facility regulations.
  • Own risk assessment and mitigation across the portfolio, including lease liability exposure, environmental compliance, and operational continuity planning.

Team Leadership & Cross-Functional Collaboration

  • Manage and develop the Real Estate Operations Administrator, with the potential to scale the team as the portfolio grows nationwide.
  • Establish proactive communication rhythms with Growth, Scheduling, TMA, Finance, and Customer Success: weekly real estate briefs, structured portfolio updates, and zero-surprise site changes for cross-functional partners.
  • Partner with FP&A on site economics modeling, with Legal on lease structures and compliance, and with Growth on aligning site expansion to physician recruitment campaigns.

Required Experience:

  • 4+ years of progressive experience in commercial real estate, portfolio management, or healthcare site strategy
  • Demonstrated track record of commercial lease negotiation with measurable cost savings, including lease restructuring, consolidation, and portfolio rationalization.
  • Experience managing a real estate or facilities budget of $3M+ annually and site-level financial analysis.
  • Proven ability to connect real estate decisions to revenue outcomes, using data-driven site selection, ROI modeling, and geographic coverage analysis.
  • Strong project management skills with experience managing concurrent build-out, renovation, and site activation projects on time and on budget.
  • Knowledge of healthcare facility compliance requirements, including ADA accessibility, OSHA safety standards, and healthcare-specific regulatory environments.
  • Excellent strategic communication and stakeholder management skills, with the ability to partner credibly with Finance, Legal, Operations, and executive leadership.
  • Proficiency with real estate analytics, lease administration platforms, and portfolio management tools.

Nice to have:

  • Bachelor's degree in Real Estate, Business Administration, Finance, or related field. MBA or Master's a plus.
  • California Real Estate License or professional certifications such as RPA, CFM, or CCIM.
  • Direct experience in the California workers' compensation or med-legal industry, including familiarity with DWC QME paneling site requirements and evaluation facility standards.
  • Experience with site access/security technology (Nexkey, Igloo, or similar smart lock/access control platforms).
  • Deep familiarity with Southern California commercial real estate markets, medical office inventory, and landlord relationships.

Work Setting:

Hybrid - Remote Home Office with regular on-site presence at flagship evaluation centers across California, primarily Southern California.


Regular travel required across California for site tours, lease negotiations, build-out oversight, and portfolio reviews (30% - 50%). Primarily within Southern California with periodic travel to Northern California and Central Valley locations.
