About the role

The Remote Care Coordinator will oversee patient care through both personalized care coordination and remote monitoring of real-time health data. This includes developing and implementing care plans, monitoring patient progress, educating patients, and collaborating with providers to ensure proactive interventions and improved health outcomes.

What you'll do

• Develop and support individualized care plans for patients with chronic conditions.
• Monitor patient data from remote devices, including blood pressure monitors, pulse oximeters, weight scales, chest sensors, and other remote monitoring tools.
• Review patient readings and symptoms over time to identify abnormal trends, changes in condition, or potential concerns.
• Escalate clinical concerns appropriately to physicians, providers, and internal care teams.
• Conduct patient assessments, check-ins, and follow-ups via phone, text, email, and other approved communication channels.
• Provide patient education on disease management, medication adherence, device usage, program participation, and lifestyle modifications such as diet, exercise, and stress management.
• Support patient onboarding and technical setup within the Monovo Clinician Portal, Monovo Patient Application, and applicable remote monitoring programs.
• Educate patients on how to successfully participate in their care program, including how to use devices, submit readings, and communicate with the care team.
• Assist in identifying and enrolling patients who qualify for Monovo’s virtual health programs, ensuring screening criteria and program requirements are met.
• Coordinate care with primary care providers, specialists, and interdisciplinary teams.
• Communicate clearly and professionally with providers by summarizing patient context, relevant trends, actions taken, concerns, and recommended next steps or requests.
• Document patient interactions, clinical data, care plans, education, follow-up, and escalation activity in the Monovo Clinician Portal and other applicable systems.
• Support patient engagement and participation in remote monitoring and care coordination programs.
• Ensure compliance with HIPAA, CCM, RPM, and other applicable program guidelines.
• Stay current on best practices in telehealth, care coordination, chronic care management, and remote patient monitoring.

Qualifications

• Active U.S. Compact RN license required.
• Minimum of 1 year of nursing experience required, preferably in ICU, Med/Surg, chronic care management, case management, primary care, population health, or a similar clinical setting.
• Strong clinical assessment and critical thinking skills, including the ability to recognize and respond to urgent changes in patient condition.
• Ability to review patient vitals, symptoms, and health data over time and identify when trends require follow-up or escalation.
• Experience with care coordination, patient education, and remote monitoring technology preferred.
• Proficiency with EHR systems, healthcare documentation, and clinical workflows.
• Strong documentation habits, including timely, objective, accurate, and complete charting.
• Excellent verbal and written communication skills, with the ability to engage patients remotely and explain clinical information in simple, patient-friendly language.
• Ability to communicate clearly and professionally with providers, internal team members, and interdisciplinary care teams.
• Strong attention to detail and ability to consistently follow clinical workflows, protocols, and escalation processes.
• Proactive follow-through with patients, providers, and internal team members.

Preferred Qualifications

• Experience working in remote patient monitoring, chronic care management, case management, population health, telehealth, or primary care.
• Experience caring for patients with chronic conditions such as hypertension, diabetes, COPD, CHF, kidney disease, obesity, or cardiovascular disease.
• Experience reviewing blood pressure, heart rate, oxygen saturation, weight, symptoms, and patient-reported concerns in a clinical context.
• Experience working with older adult populations and patients who may need additional education, encouragement, or technical support.
• Comfort balancing clinical judgment with established workflows, protocols, and escalation criteria.
• Experience troubleshooting basic patient technology issues, including device setup, app access, Bluetooth pairing, connectivity issues, and patient login support.
• Experience supporting patient onboarding, enrollment, or program participation in a healthcare setting.
• Experience using multiple software systems throughout the day while maintaining accuracy and efficiency.
• Bilingual abilities are a plus.
• Prior experience working in a startup, small team, or fast-changing healthcare environment is a plus.

Requirements

• Commitment to a minimum of 20 hours/week during business hours across mixed timezones
• U.S. work authorization
• Must reside in UT or TX
• Must be able to work at a computer for extended periods of time, including viewing a screen, typing, navigating multiple systems, and remaining seated for much of the workday.
• Tech-savvy and comfortable learning new systems, using multiple platforms at once.
• Must maintain a private, HIPAA-compliant workspace at home where patient information can be discussed, viewed, and documented confidentially.
• Must have a reliable high-speed internet connection that supports video calls, phone calls, EHR access, remote monitoring platforms, and other required systems.
