On Belay Health Solutions is a Massachusetts-based physician-led managed services organization (MSO) devoted to supporting independent primary care physicians. On Belay provides enhanced Medicare value-based contracts that empower physicians with the resources necessary to invest in innovative care models and population health management tools. About the role
- On Belay is seeking a motivated, passionate Manager of Practice Transformation, who will be responsible for working closely with our Provider partners and their staff to improve the quality and lower the cost of care they provide to Medicare beneficiaries.
What you'll do
- Identify, develop, and implement suggestions for provider practice improvement plans and assist providers in developing, implementing, monitoring, and tracking of improvement activities that improve performance in value-based contracts
- Utilize professional quality improvement experience and expertise to guide the practice team members in achieving project-specific goals and timelines
- Work with On Belay’s network and practice team members to facilitate development and adoption of effective monitoring tools to track progress toward goal-oriented plans
- Prepares and delivers reports with actionable opportunities to practices on a routine basis
- Develops a deep relationship with practice staff and providers to be seen as a trusted resource for value-based care operations
- Access and incorporate available resources, tools, and/or materials (IT, analytic, clinical program and field education) to support transformation activities and clearly articulate and communicate areas of concern, needed tools, or barriers to practice achievement of goals to network leadership and other project team members
- Proactively communicate and problem-solve with the practice for efficient and effective progression of improvement initiatives
- Partner with the practice leadership and team members to identify and support standard improvement activities across practices as appropriate to improve network-wide performance
- Participate in sharing and learning related to improvement initiatives and other similar activities
- Identify opportunities for improvement of processes in which multiple team members are involved
- Establish successful working relationships and interact effectively with colleagues, leadership, committee and board members, providers, practice managers, community partners, management, and other staff to solicit input and buy-in from clinical program customers/end users and staff
- Participate on various practice and organizational transformation project teams as assigned
- This role is a hybrid with the expectation to work with practices both in person and virtually
- Perform other duties as assigned
Qualifications
- 3+ years working directly with primary care practices on population health and value based care initiatives for either a payer, MSO, or similar
- 2+ years of applicable experience as a medical office manager and/or lead administrator of a primary care or family practice preferred
- Deep knowledge of Medicare Advantage, commercial, and government operated value based care contracts
- Successful track record of engagement with providers including a thorough understanding of their organizational structure and workflow management
- Related experience working in the healthcare industry (i.e., health plan, hospital, government, actuarial or management consulting) with a proven track record of successfully engaging with providers including a thorough understanding of their workflows and organization
- Proven ability to lead, coordinate, and facilitate new ideas into action while collaborating across multiple dynamic projects and teams, removing obstacles to achieve desired outcomes
- Outstanding emotional intelligence and ability to build the trust and confidence of a wide range of stakeholders
- Strong leadership skills and strong influencing skills
- High level of organization and detail orientation
- Quick learner comfortable with a high level of ambiguity
- Bachelor degree required

Compensation
- We offer a competitive base salary ranging from $115,000 to $125,000 annually, depending on experience.
