About the role

Care Navigators will work with community members, including older adults and those from diverse and underserved communities, to assess and address health and social service needs and promote well-being. Care Navigators will utilize a comprehensive assessment and a range of peer coaching strategies, including health education activities, friendly check-ins, and resource linking, all by phone. Other duties may include community outreach and evaluation.

What you'll do

  • Participate in new team member orientation, as well as ongoing training
  • Clearly and efficiently follow provided processes and policies
  • Be prompt and thorough with scheduled engagements, follow-through,  documentation, and tracking progress
  • Use your discretion and keep conversation topics appropriate
  • Converse in a friendly and engaged manner and show empathy and discretion by building rapport and creating a positive and welcoming experience for community members
  • Have a good sense of community and available resources available to community members to access
  • Utilize community resources, social services, and partnerships to meet community members' needs. 
  • Able to communicate effectively by relaying questions and concerns to staff and promptly responding to team-wide communications
  • Have cultural humility, knowledge, and awareness of community cultures and values
  • Other duties as assigned

Qualifications

  • MUST BE LOCATED IN SANTA CLARA COUNTY
  • Applicant must meet one of the following minimum qualifications for CHWs in California:
    • Certificate Pathway: A valid certificate of completion of a curriculum that attests to demonstrated skills and/or practical training in the following areas: communication, interpersonal and relationship building, service coordination and navigation, capacity building, advocacy, education and facilitation, individual and community assessment, professional skills and conduct, outreach, evaluation and research, and basic knowledge in public health principles and social drivers of health (SDOH).
    • Work Experience Pathway: An individual who has at least 2,000 hours working as a CHW in paid or volunteer positions within the previous three years and has demonstrated skills and practical training in the areas described above
  • Reliable transportation and a flexible schedule for face to face outreach.
  • Excellent written and verbal communication skills; comfortable communicating and engaging in person or by video conference and phone with staff, community members, family members, and community partners
  • Strong computer skills; ability to learn and operate web-based systems effectively (state-required programs)
  • Confidentiality when dealing with confidential matters or sensitive subjects by abiding by all HIPPA guidelines
  • Highly organized with attention to detail
  • Able to maintain professionalism and flexibility in times of change and improvement
  • Flexible schedule
