About the Position

3M’s Services LLC is seeking an experienced and motivated Qualified Professional to join our growing team.

The QP plays an important role in ensuring individuals with Intellectual Developmental Disabilities (I/DD) receive high-quality, person-centered services that promote independence, community involvement, choice, dignity, and personal growth.

This position is ideal for a human services professional who enjoys a combination of caseload management, working in the community, supporting direct care staff, collaborating with families and Care Managers, and ensuring services remain compliant and effective. The QP will manage an assigned caseload and work closely with the Program Director, Direct Support Professionals, families/guardians, Care Managers, and other members of the individual's support team.

What You'll Do

• Manage and oversee an assigned caseload of individuals receiving community-based services.
• Coordinate and monitor services according to each individual's ISP/Plan of Care and authorized services.
• Participate in ISP meetings, treatment team meetings, and other person-centered planning meetings.
• Maintain regular communication with individuals, families/guardians, Care Managers, and Executive Leadership.
• Complete required member monitoring and community/site visits.
• Monitor individual goals, progress, service utilization, and overall quality of services.
• Supervise, coach, and support Direct Support Professionals assigned to your caseload.
• Conduct staff observations and required supervision visits.
• Train staff on individual goals, support needs, documentation expectations, and service requirements.
• Review service documentation for accuracy, quality, and timely completion.
• Monitor authorizations, consents, assessments, and other required documentation.
• Monitor EVV compliance for applicable services.
• Identify staffing or service-delivery barriers and work with the appropriate departments to resolve them.
• Assist with interviewing, onboarding, and matching DSPs with individuals when needed.
• Ensure services meet applicable Medicaid, LME/MCO, state, and agency requirements.
• Identify and promptly communicate health, safety, service, or compliance concerns to the Program Director.
• Participate in audits, quality improvement activities, staff meetings, and required training.
• Maintain organized, accurate, and audit-ready records for assigned cases.

What We're Looking For

We're looking for someone who is organized, dependable, proactive, and comfortable taking ownership of their caseload.

The ideal candidate can successfully balance documentation and administrative responsibilities with community-based work and relationship building.

You should have:

• Min of 2years experience working with individuals with I/DD and/or behavioral health needs.
• Strong caseload and time-management skills.
• Excellent written and verbal communication skills.
• Experience supervising or coaching direct care staff.
• Strong documentation and organizational skills.
• Ability to work independently and meet established deadlines.
• Ability to identify problems and follow through until they are resolved.
• Professional communication skills when working with families, guardians, staff, and external stakeholders.
• Basic proficiency with electronic documentation systems, email, and Microsoft Office/Google Workspace.
• Reliable transportation and the ability to travel throughout assigned service areas.

Qualified Professional Requirements

Candidates must meet applicable North Carolina Qualified Professional minimum requirements, including one of the following combinations of education and experience:

• Master's degree in a human services field with at least one (1) year of qualifying supervised MH/DD/SA experience;

OR

• Bachelor's degree in a human services field with at least two (2) years of qualifying supervised MH/DD/SA experience;

OR

• Bachelor's degree in a non-human services field with at least four (4) years of qualifying supervised MH/DD/SA experience;

OR

• An applicable professional license, certificate, registration, or permit with the qualifying experience required for the population served.

Education and experience will be verified before placement into the Qualified Professional position.

Required Qualifications

• Valid driver's license and reliable transportation
• Ability to successfully complete required background and registry screenings
• CPR/First Aid certification or ability to obtain
• NCI/crisis prevention certification or ability to obtain
• Medication Administration certification, when applicable
• Ability to complete all required 3M’s Services LLC orientation and training requirements

Hybrid Work Schedule

This position offers a combination of office, community, and remote work.

New QPs will primarily work on-site during the first 90 days to complete training, orientation, caseload development, and competency requirements.
